The clinical team at the RVC analysed anonymised clinical data from dogs from the UK, Europe and the USA undergoing mitral valve repair at the Queen Mother Hospital for Animals at the RVC.
One study evaluated the anaesthetic management and outcomes of 176 dogs undergoing surgery, focusing on complications during and around the time of anaesthesia1.
The second study assessed 66 dogs to identify features on heart scans that could help predict whether the heart would remain enlarged after surgery2.
The findings showed that mitral valve repair is associated with significant intraoperative and postoperative risks.
Longer cardiopulmonary bypass duration was associated with increased risk.
Dogs with greater pre-surgical heart enlargement and higher body weight were more likely to have persistent left heart enlargement after surgery.
Close perioperative monitoring and rapid-response protocols were found to be essential in managing anticipated complications.
The study also found that recovery outcomes vary, meaning individual patient factors must be considered when advising owners.
Finally, the research demonstrated that most beneficial changes in heart size, known as reverse remodelling, occurred within the first three months after surgery.

The procedure, called transcatheter edge-to-edge repair (TEER), is a minimally invasive alternative to open heart surgery.
TEER is performed on a beating heart using a specially designed “V-Clamp” device which brings the mitral valve leaflets together, reducing the severity of mitral regurgitation.
The procedure is guided by real-time 3D echocardiography and performed via a small thoracotomy, typically 6–8 cm in length.
CVS, which owns Bristol Vet Specialists, says dogs undergoing TEER generally experience a rapid recovery, with most discharged within 48 hours and minimal need for intensive care.
The company says that once heart failure is diagnosed, the average survival time is between nine and 12 months, but that after TEER surgery, 95% of dogs are discharged successfully, and 85% survive for two years post surgery.

The model demonstrates the structural changes in the mitral valve and associated heart chambers across stages A to D — from dogs at risk (stage A), through preclinical disease (stages B1 & B2), to symptomatic heart failure (stages C & D).
It has been designed to help pet owners understand the condition’s progression and support clearer communication of monitoring and treatment options.

The company says the two main benefits of the liquid formulation are precise dosing and ease of administration.
Vetmedin oral solution comes with a kg-bodyweight calibrated syringe to make it easy to give the preferred dose of 0.25 mg/kg twice daily to the nearest 0.5 kg bodyweight.
The maximum syringe volume is for a 12 kg dog.
The solution has a sweet taste to make it palatable and Boehringer says it is also ideal for dogs that need dental work but require treatment for their heart disease before general anaesthesia.
These dogs might struggle to eat chewable tablets due to oral discomfort, so a liquid treatment might be a better option.
The company highlights the EPIC1 and PROTECT2 studies which showed that Vetmedin delays the onset of CHF for dogs in the asymptomatic stage of heart disease and improves both the quality and length of life for dogs with MVD and DCM.
Vetmedin oral solution comes as a 50 ml bottle (each bottle lasts a 5 kg dog approximately 30 days), doesn't need to be refrigerated, has an 8-week shelf life once opened, and a 2-year shelf life as packaged.
VetMedin oral solution is now available to order from wholesalers.
The project looked first at how effective CVS small animal first opinion practices were at conducting echocardiography.
It found that a barrier to wider uptake was a lack of up-to-date equipment, along with a lack of current knowledge and training - both in using ultrasound and echocardiography equipment and in interpreting images.
CVS says that as a result, many colleagues were routinely referring suspected cardiac disease cases.
The project leaders then reviewed and recommended practice equipment, so that clinicians had the tools to perform echocardiograms in practice.
This included buying echocardiography tables - so that practitioners could scan an animal’s chest from underneath, more advanced cardiac ultrasound machines and electrocardiogram (ECG) machines.
The company then produced a range of resources for vets and nurses, to help them to improve echocardiography scan rates and nurse cardiology clinics, including clinical guidelines, machine guides and client resources.
It then held a number of training sessions for its vets and nurses, focusing on the latest advancements in heart disease diagnosis and treatment for vets, and training nurses how to conduct nurse cardiology clinics, how to perform and interpret electrocardiography, how to take blood pressure readings and thoracic point of care ultrasound (POCUS).
Lynsay Morgan, Regional Clinical Lead at CVS who is responsible for the cardiac ultrasound clinical improvement project, said: “We want to empower our first opinion small animal vets to perform more echo evaluations and to work up their cardiology cases to a high standard.
"We also want our nurses to be involved in patients’ life-long care, and provide ongoing support to owners of pets with heart disease.
“We hope this project will improve case identification, increase skills and confidence, and see optimum diagnosis and management of cardiac disease.”
The pair were invited to carry out the laparoscopic surgery on four dogs suffering from pulmonic stenosis by the British Veterinary Centre.
Chris said: “The British Veterinary Clinic had previously referred a case to Fabio at Willows and subsequently got in touch about a further four cases in need of surgery, inviting us to fly out to Abu Dhabi to operate on these cases for them.
“It was a pioneering moment for the British Veterinary Centre and a first for the region and the Middle East.
“It’s the very first time that interventional cardiology has been performed in the UAE so, with the fantastic facilities and dedicated staff at the BVC, we have made history in the region!
“It was also the first time that Fabio and I had worked together and now we hope that this will be the first of many such collaborations.”
Fabio added: “With our collective expertise and knowledge, we know we can improve the lives of so many dogs that are living with conditions like pulmonic stenosis.
“Two of the dogs required balloon valvuloplasty and two required pulmonary stents inserted.
“All four dogs had great initial post-operative results and would be expected to lead happier, healthier lives as a result of these procedures.'
Furosoral contains furosemide and is indicated for the treatment of hydrothorax, hydropericardium, ascites and oedema, particularly in animals where these conditions are associated with cardiac insufficiency and renal dysfunction.
Furosoral replaces Dechra’s previous diuretic treatment, Frusedale.
It is available in both 10 mg and 40 mg double-divisable tablets.
The recommended daily starting dose for cats and dogs is 2.5 mg per kg of body weight.
Dechra says careful weighing is essential to ensure accurate dosing, but that the dosage can be doubled initially for severe or particularly resistant cases and adjusted to the lowest clinically effective dose for maintenance therapy if required.

The recommended starting dose is 0.125-0.25 mg amlodipine per kg bodyweight per day, which equates to half a tablet for cats weighing less than 4kg (and more than 2kg) and one tablet per day for cats weighing between 4kg and 8kg.

The company’s NEMO Cardiac Health Screening test detects heart disease using AI to analyse microRNAs - biomarkers that manage the immune system and its responses and act as regulators for disease progression or resolution.
MI:RNA says the new test, which can be taken from blood or urine samples, will help vets diagnose and monitor disease progression more effectively and give cats and dogs with heart disease 18 months more life.

“I think the MI:RNA panels are most useful for vets in primary care practice to identify and confirm the severity of underlying cardiac disease in their patients.
"A simple blood test which could confirm that the cause of a heart murmur is MVD, and also indicate its severity, and whether the patient would benefit from treatment would be really helpful for primary care vets in practice.
"They might indicate which patients may benefit from referral, or indicate when a specific treatment is indicated.
"Future research with miRNA panels may also indicate novel pathways in progression of disease which might increase our understanding of the disease, or novel therapeutic options.”
The procedure involves introducing a catheter through the jugular, then using fluoroscopy and transoesophageal echocardiography to guide it from the right atrium through to the left side of the heart to relieve abnormal high pressures in the left atrium.
Willows says it is one of only a few centres across Europe equipped to carry out the procedure and that typically, patients who undergo TSP can be discharged from hospital the day after surgery.
Fabio Sarcinella (pictured right), an RCVS and European specialist in small animal cardiology at Willows, said: “Early clinical evaluation of the TSP procedure in humans over the last few years has shown improved quality of life and reduced clinical signs in patients with heart failure.
“The procedure has also been associated with low-risk and a meaningful drop in left atrial pressure of the affected patients.”
Fabio added: “As well as being minimally invasive, the improvement in heart chamber pressures via TSP often allows for a reduction in the dose of water tablets which are used to control the heart failure signs in the lungs.
"Lowering the dose reduces the risk of side effects related to these drugs such as kidney failure.”
Willows says TSP is most commonly used for left atrial decompression in dogs with heart failure and concurrent renal disease or that have advanced heart failure but continue to have symptoms despite optimal medical treatment.

The survey of 1,019 dog owners was conducted by petbuzz in December 2022.
Veterinary cardiology specialist, Dr Rachel James, said: “The CKCS has long been a very popular breed of dog in high demand, due to their loving and placid nature making them perfect family pet.
"We know that sadly at least 50% of CKCS have evidence of MVD disease by the time they are five, with 98% of dogs affected in their lifetimes.
"However, we can help our clients when selecting this breed to choose a puppy that is less likely to develop MVD at an early age, by utilising the heart testing scheme (Doppler heart testing).
"Prospective owners should be asking to see test results for the parents, grandparents, and even better great grandparents too, on both maternal and paternal sides.
"In addition breeders need more support and education to encourage the use of the heart testing scheme and how this can enable them to choose the best dogs to breed from.
"Furthermore we should be encouraging breeders to breed from dogs only when they are at least 3 years of age.”
Vita Animal Health commissioned the survey to raise awareness of the condition and to remind owners to use their vet’s expertise.
Vita’s veterinary nurse, Tara Evans, said: “The CKCS is a very popular breed of dog, perhaps set to be more so now King Charles III is reigning monarch.
"This survey has shown that awareness of mitral valve disease – which is very common in this breed – is not as good as it could be.
"We want to support vets in raising awareness and have useful resources such as our heart disease infographic (www.vitaanimalhealth.com/common-signs-of-heart-disease) that we welcome all vets in using.”
Willows says it is currently the only centre in the UK to offer the procedure, and one of very few in the world.
Simon Swift, an RCVS and European specialist in small animal cardiology (pictured right), heads the cardiology team at Willows.
He said: “Transcatheter edge-to-edge mitral valve repair (TEER) is already considered an alternative to surgical repair in human patients and there have been some very promising early clinical results in dogs.
“The procedure is performed under general anaesthesia through a small incision in the chest wall.
“Access within the beating heart is achieved by a needle puncture at the apex of the heart which is highlighted by continuous X-ray images and an ultrasound of the heart via a probe placed in the oesophagus.
“This allows correct positioning of a V-clamp across the mitral valve to reduce the amount of leakage.
“It’s a procedure that delivers meaningful results and an improved quality of life and, because it’s a minimally invasive process, patients are typically discharged from hospital within two days.”

The referral centre has invested in a new heart and lung machine, and an oxygen cage, and is expected to take its first patients in the autumn.
The service will be established and led by head of cardiac surgery Poppy Bristow (pictured right) and head of cardiology Anne Kurosawa, who have already performed this kind of surgery on more than 100 dogs to date.
DWR is also setting up its own blood transfusion service to collect blood from staff and clients’ pets to support the service.
The blood collected will also be used to support the practice’s emergency and critical care service and its planned dialysis programme.

CardioCare is based on research carried out at the Purina Technology Center which explored how nutrients can support the ailing heart. First, a series of metabolomics studies identified metabolic changes that occur in MMVD. Then the company developed a ‘Cardiac Nutritional Blend’ to address these metabolic changes and evaluated it in a clinical study.
The main elements of the Cardiac Nutritional Blend include carnitine precursors, antioxidants and medium-chain triglycerides. Purina says the latter provides an alternative energy source that is easy for the struggling heart to use, something which is particularly important as the metabolomics research revealed that dogs with MMVD showed less efficient cardiac energy metabolism1.
The company highlights a 2019 clinical study, which showed that the Cardiac Nutritional Blend has the potential to delay disease progression in dogs with preclinical MMVD. Over the six-month study period, dogs with preclinical MMVD fed the Cardiac Nutritional Blend showed a reduction in left atrial size, while an equivalent group of dogs fed a control diet showed an increase. Additionally, none of the dogs fed the Cardiac Nutritional Blend progressed from stage B1 to B2 during the study, while 37% of dogs fed the control diet showed progression2.
Purina also says that while the research to date has focused on MMVD, the nutrients in CardioCare are recognised to be beneficial under conditions of cardiac stress and the diet may prove a useful tool to provide support in a wider range of conditions. The company recommends that the diet be considered as part of the management plan for any dog with cardiac insufficiency.

The company points to research which shows that most dogs and cats whose congestive heart failure is well controlled by medication will have a stable sleeping respiratory rate of less than 30 breaths per minute1,2.
Vetoquinol says SRR is a sensitive indicator of pulmonary oedema or pulmonary effusion which can occur as heart failure patients deteriorate, and that monitoring of SRR is a useful biomarker for veterinary surgeons, allowing you to track progression of congestive heart failure and to detect stability during treatment.
The company also says SRR is most easily detected by the owner when the pet is asleep: using the app to count and record SRR gives clients a good indication of when to seek veterinary intervention and generates data that helps veterinary teams seeking to minimise unnecessary face-to-face contact but still provide emergency care.
While an SRR in the reference range does not exclude heart disease, it reduces the probability that the patient is suffering from left sided heart failure, so may also be a useful measure for remote triage as part of a wider assessment. Other causes of elevated SRR include pain, pneumonia, stress and heatstroke.

Ceva says the course is aimed at vets and nurses who want to build their confidence and take the lead in finding hypertension in cats in their clinic.
The course is hosted by Sarah Caney BVSc DSAM(Feline) MRCVS, one of the UK's leading specialists in feline medicine, and will cover things like finding out which cats need screening for feline hypertension, how to become confident in recommending a blood pressure check to owners, hints and tips on taking a reliable blood pressure measurement and handling cats calmly to reduce ‘situational hypertension’.

The clinic will assess cats and dogs under six months old where a heart murmur has been detected.
Willows will then recommend a course of action and, if required, carry out keyhole surgery.
Chris Linney, head of cardiology at Willows, said: "This is the first specialist-led, congenital heart disease clinic in the country and will use some of the latest and most advanced equipment available in the veterinary world. "All young animals with murmurs may benefit from the clinic, but those that are always recommended to undergo assessment are puppies with murmurs grade three (out of six) and over, and kittens with murmurs grade four (out of six) and over.
"By reviewing these animals at a younger age, we hope to be able to put care plans in place that can improve their long-term health.
"We will also offer a precise diagnosis, outlook and prognosis on the cause of the heart murmur and, where needed, carry out minimally-invasive keyhole surgery.
"Often, the earlier these are performed the better the outlook for the patient."

The stem cells were created as part of a study into feline hypertrophic cardiomyopathy (HCM), which the RVC carried out in association with the Animal Health Trust (AHT), The Beryl Evetts and Robert Luff Animal Welfare Trust (BERLAWT), The Winn Feline Foundation and Boehringer Ingelheim.
The RVC says HCM affects about 15% of the feline population in the UK; translating to over 1 million cats. It is caused by genetic mutations which affect the heart muscle cells.
To date, there are no treatments proven to stop or reverse it, leading to a very poor prognosis for affected cats.
This is partly because a major issue with studying heart diseases at a cellular level is that heart muscle cells do not survive in a laboratory environment. When studying human heart cells, this has been overcome by turning ordinary skin cells into pluripotent stem cells (cells able to turn into any tissue in the body).
These are termed 'induced pluripotent stem cells', or iPSCs. It is then possible to generate heart muscle cells to study from these iPSCs.
The research, carried out by Dr Luke Dutton under the supervision of Professor David Connolly, Dr Jayesh Dudhia and Dr Debbie Guest, as part of his PhD at the RVC in collaboration with AHT, has now identified the conditions required to create iPSCs from domestic cat cells.
While creating iPSCs has been done before in wild cats, the RVC believes that Dr Dutton’s breakthrough in creating iPSCs from domestic cat cells is a world first. It sets the scene for further research into how to turn these iPSCs into heart cells and then, once successful, testing drug therapies that could improve outcomes for cats with HCM.
The researchers say that the translational benefits of this project are potentially very significant, as around 1 in 500 people in the UK have HCM, and the condition manifests in humans in the same way.
If the therapies that researchers test on feline heart cells turn out to be effective, this sets the stage for testing these treatments on humans.
Dr Dutton, author of the study, said: "This is an incredibly exciting project, which is only made possible by the generous funding of the BERLAWT, Boehringer Ingelheim and The Winn Feline Foundation.
"Not only is this study the first reported generation of iPSCs from domestic cats, but these cells can now be used in a novel disease model. This will allow us to study the disease processes present in these cats in ways that have not been possible with the ultimate goal of identifying new therapeutic agents that may slow or even stop the disease process. We would then hope to translate these agents into the feline clinic."
Dr Debbie Guest, Head of Stem Cell Research at the Animal Health Trust, said: "This is the first report on the successful generation of domestic cat iPSCs. These cells not only bring hope to cats suffering from HCM, but allow us to develop new tools to study conditions affecting other tissue types in the future."
The new drug represents the fruit of 8 years of product development and more than 11 proprietary studies performed by Ceva R&D teams.
Marie Boutot, development project leader said: "Isemid is an original drug developed by Ceva, and was the subject of full pivotal efficacy and safety studies."
Ceva says the new drug has been recognised by the European Medicines Agency as a "2018 Innovation, advancing animal health".
Isemid is presented as pork-flavoured, scored tablets in three strengths: 1, 2 and 4 mg. They can be administered orally once daily, with or without food. The company's research showed a 98% owner compliance (compared to 92% for furosemide, p<0.05) and 80% voluntary acceptance.

Scientific communications with data on Isemid were presented at the American College of Veterinary Internal Medicine forum in Phoenix in June and will be held at the European College of Veterinary Internal Medicine congress in Milan in September.
