RCVS Council had introduced temporary guidance allowing the remote prescription of drugs for animals not under care back in March, to ensure that animal health and welfare could be maintained during lockdown without risking the health of veterinary teams or their clients.
Since then, the College has twice extended this guidance, because of the ongoing situation.
However the College says it now recognises that many practices are returning more to 'business as usual' and that the guidance and associated flowchart should be updated according.
Consequently, before deciding to prescribe POM-Vs remotely, the updated guidance now requires veterinary surgeons to first consider whether the animal is already under their care; or, if not, whether it is possible to physically examine the animal in order to bring the animal under their care. If the answer to both questions is ‘no’, POM-Vs may still be prescribed remotely providing the guidelines set out in the College’s coronavirus advice hub are adhered to.
Surprisingly, the College says that its surveys of the profession have thus far identified no immediate safety concerns around remote prescribing.
RCVS President Dr Mandisa Greene, who chairs the Taskforce, said: “The reason for maintaining the possibility of remote prescribing without a physical examination was that we recognised that the current situation is unpredictable, and while the ability for the public to visit practices in person has improved over the last few months, we felt that situations might still arise where that would not be possible, and where access to remote prescribing would be necessary. These could include further local lockdowns, ongoing quarantine arrangements, and the remaining fact that some members of both the veterinary team and the public continue to shield.
“It remains our intention that this guidance will continue to be a temporary measure and may be subject to further extensions or updates given the uncertain nature of the Covid-19 pandemic.”
RCVS Council will review the position on 8 October, with any changes being effective by 1 November at the earliest.
Meanwhile, the RCVS review of ‘under care’ and out-of-hours emergency cover has now resumed, starting with a number of virtual focus groups and consultation with stakeholders within the veterinary and veterinary nursing professions.
The findings from these focus group discussions will then inform a wider survey to be sent to all veterinary surgeons and veterinary nurses in early 2021, along with stakeholder organisations and the animal-owning public. Remote prescribing will continue to form a part of this review.
As part of the redevelopment, kennel capacity will increase from 107 to 197, there'll be a new emergency and critical care hub, sixteen new consult rooms and a new imaging suite with MRI and CT scanners, two ultrasounds and two X-ray machines.
There will also be a 66-seat seminar room.
Work is expected to be completed early next year, and the practice will be recruiting for around 75 new team members to service the expanded centre.
Simon Tappin, clinical director at DWR, said: “This long-awaited expansion will transform our practice, offering even more space and capacity for referrals, brand-new equipment and a seamless experience for our clients.
“It will also bolster our reputation as a training practice, with the expansion of our intern and residency programmes adding to our already-impressive provision in this area."
The practice is also implementing case management changes, with a single point of contact for every client and individual case managers in every discipline.
The traditional reception area is being replaced with a concierge greeting service for clients, with staff on hand to book them in on iPads.
Client care team manager Deborah Bell said: “This is a USP for us and we think it’s an industry first. The idea came from how medical secretaries work in human health.
“When a referring vet calls us, our client care team takes the basic details and passes them on to the case manager for that discipline. They will contact the client, make appointments and meet the client in person on arrival.
“We have 12 case managers for our disciplines at the moment, with the potential to expand. As well as providing a personalised service for our clients, it provides great career progression opportunities for our client care team.”

The response offers further evidence concerning the CMA’s potential concerns, and says that voluntary measures relating to transparency of fees and ownership could be introduced before the conclusion of a market investigation.
The BVA has formed a working group to develop guidance on transparency, client choice and the value of veterinary care to support companion animal practices.
British Veterinary Association President Anna Judson, said: “We’re keen to see healthy competition and consumer choice and we recognise that some of the areas identified by the CMA would benefit from further scrutiny.
"We are already taking steps to support vet practices by developing guidance for the profession in relation to transparency of fees, transparency of ownership, prescribing and dispensing of veterinary medicines, and contextualised care.
“However, we are extremely disappointed to see the suggestion in the CMA’s report, as well as in subsequent media reporting, that veterinary professionals might prey on owners’ desire to do the best for their pets by using these circumstances as a “strategy” to promote more sophisticated or expensive treatment.
"Vets enter this high-pressure profession out of genuine care for animals and will always prioritise their health and welfare."
“We also have significant concerns about the suggestion that practices might be mandated to provide information to clients about quality or outcome-related measures.
"We do not see primary concerns about the quality of veterinary treatment and we believe such a requirement would not meaningfully support consumer choice.
"Instead, it would be extremely challenging to deliver and could significantly exacerbate many of the complex issues faced by our members, who are already under significant pressure.

COMMENT
There are lots of good points made in this latest response to the CMA, if no more than a nod to the real issue here: which is the often eye-watering cost of modern veterinary care.
But it is hugely disappointing to see the BVA (and others) berating the CMA and the media for suggesting that vets might prey on owners' desire to do the best for their pet, rather than asking WHY people think that, and how can the profession regain trust.
That, surely, is the starting point here: for the profession to stop feeling sorry for itself, and instead think how it can change to meet the needs of its customers and dispel their perception of being taken for a ride.
The CMA Investigation is a bit of a red herring: the price of veterinary care is not going to be significantly changed by displaying the prices for a few standard procedures on practice websites, with greater transparency over practice ownership or with slightly cheaper prescriptions, all of which is just tinkering.
The biggest influence on the size of the client's bill is not marketplace competition, it's the vet's recommendation.
Paul Manktelow, Chief Vet at the Blue Cross has already hit the nail square on the head when he called on the profession to practise pragmatic medicine.
The BVA now alludes to this with talk in its response of 'contextualised care' - a concept which allows practitioners to offer pragmatic medicine (aka what people can afford, rather than 'gold standard'), without the judgement that it is inherently associated with).

That's great, but will it be enough?
Perhaps not, because whilst someone might be happy to walk out of the car showroom with a Dacia Sandero knowing they cannot afford a Bentley, they may be less happy to accept the Dacia level of care for their beloved pet, when they watched 'Supervet' only the night before.
If that's true, then pragmatic medicine will have to be combined with all sorts of other measures, such as reviewing and reducing the cost of regulation, questioning the value of diagnostics and changing to a less 'referall' (sic) culture.

The figures came from an analysis of over 100,000 calls made to VidiVet by owners when their normal practice was closed, between 2021 and 2024.
So far in 2024 36% of VidiVet’s calls have been gastrointestinal cases, 25% skin cases, 14% musculoskeletal and 3.7% post-surgical issues.
The advice given to many of these cases was that an emergency visit to an out-of-hours (OOH) clinic wasn’t necessary, but to monitor at home then attend their own vet practice for any workup, diagnostic tests, medication etc.
The company is now encouraging practices to consider using it's triage service over Christmas, when practices and emergency clinics often experience an increase in OOH workload.
This, it says, is a win-win situation as it relieves pressure on OOH clinics, brings revenue back to veterinary practices and also saves clients money, stress and time.

For the study1, the authors Lucy Norris, Gina Pinchbeck, Peter-John Noble and Alan Radford, analysed data from electronic health records submitted by veterinary practices participating in the Small Animal Veterinary Surveillance Network (SAVSNET).
They found that a total of 132 dogs with cropped ears were seen by vets across a sample size of 500 practices between 2015 and 2022, with numbers peaking in 2021.
The breeds most likely to have cropped ears were the American bulldog, Dobermann, Italian mastiff and bulldog.
In more than 60% of the cases, the dogs had been imported from countries where ear cropping is illegal, such as Romania, Hungary, Bulgaria, Serbia, Spain, Poland and Ireland.
The authors acknowledge that the findings of their research are likely just a small snapshot of the true scale of the ear cropping problem: figures from the 2022 PDSA PAW Report suggest that the true numbers of dogs imported with cropped ears could be as high as 26,000.
RSPCA Head of Companion Animals Samantha Gaines said: "The study provides strong evidence to refute arguments for ear cropping to prevent ear infections, as the breed types identified in Norris and colleagues’ study are not generally predisposed to such infections.
"Rather, this finding adds weight to anecdotal evidence suggesting that the motivation for ear cropping in many dogs is driven by appearance rather than any perceived health benefits.
“It is also possible that some dogs were cropped illegally in the UK using legal importation as a smokescreen for this activity.
"Indeed, it is the RSPCA's experience that many dogs stated as being cropped outside of the UK have histories that would make this highly unlikely.”
Vet Record editor-in-chief Suzanne Jarvis said: “One solution to the importation problem is proposed in the Kept Animals Bill, which is currently stalled in parliament.
"Although there could be potential for fraud around grandfather rights initially, a ban on such importations would ultimately mean that the sight of a dog with cropped ears becomes a thing of the past in the UK.”

The grant has been awarded to Dr Carri Westgarth, a Lecturer in Human-Animal Interaction at the University of Liverpool, for her project entitled Using citizen science to develop a ‘Canine Ageing and Wellbeing’ tool for use in veterinary practice.
Dr Westgarth’s initiative aims to determine what senior dog preventative healthcare and treatment advice is currently offered in UK veterinary practices and then take steps to build a practical tool to facilitate knowledge-sharing between owners and veterinary professionals.
Carri said: "Dogs are living up to twice as long as they did 40 years ago and there are implications for senior dogs’ healthcare and wellbeing.
"As life expectancy increases, so does the amount of time spent in poor health. Many dog owners may not be aware of the signs of serious age-related diseases as they attribute them to normal age-related changes."
The Citizen Science project will investigate owner expectations, experiences and attitudes to ageing in dogs, including preventative care and general understanding of normal and abnormal changes during ageing.
A number of in-depth interviews and questionnaire surveys with veterinary professionals and dog owners will be conducted, and pet owners will be invited to submit diaries, photographs or videos to share their experiences of living with an older dog. The study will also examine electronic health records of senior dogs reported in the Small Animal Veterinary Surveillance Network.
The findings will be used to develop the Petsavers Canine Ageing and Wellbeing tool, to guide on best practice discussions in consultations with senior dogs.
She has been awarded a place on the company’s Nurse Certificate in Emergency and Critical Care (NCert ECC) programme, which starts in December 2021.
Louise O’Dwyer was a highly respected and awarded ECC veterinary nurse, who was passionate about ECC and an inspiration to her colleagues. She lectured not only on Improve International’s NCert ECC programme, but also on its other nursing courses and at congresses around the world. She died in 2019.
Jennifer said: “I am thrilled and shocked to have been selected as the winner of the Louise O'Dwyer legacy scholarship.
“Louise has been a massive inspiration and role model to me throughout my career. When I started veterinary nursing, a lecture of hers was one of the first I attended and I felt empowered through her passion and knowledge to further my own career in a similar way. I have been lucky enough to be able to follow my interests in veterinary nursing anaesthesia but have always had a passion for ECC. Receiving this Scholarship will enable me to further my interest and training in this field. I want to thank her family, together with Improve International, for this amazing opportunity.”

The Association says it delayed this decision as late as possible, and whilst the most recent government advice does not prohibit large public gatherings, it recognises the importance of personal safety for Congress attendees and the need to support the profession in preserving business continuity, maintaining client services and protecting animal welfare.

One of the consequences of veterinary price inflation has been a backlash from owners against vets, vet nurses and receptionists, so the first film kicks off by explaining why large vet bills are not the fault of the vet standing in front of them when their dog gets ill.

RVC researchers say that the VetCompass study is the largest ever carried out into dental disease in cats using veterinary clinical records.
The study investigated a sample of 18,249 cats randomly selected from a study population of 1,255,130 cats in the UK in 2019.
Periodontal disease was recorded in 15.2% of the sample making it the most common disorder.
The cat breeds with the highest annual levels of this diagnosis were Siamese (18.7%), Maine Coon (16.7%) and British Short Hair (15.5%) as well as crossbreeds (15.4%).
The average bodyweight of cats with periodontal disease (5.7kg) was higher than for cats without periodontal disease (5.5kg).
The risk of periodontal disease rose steeply as cats got older, with cats aged 9 to 12 years being 6.7 times more likely to have periodontal disease compared with cats aged under 3 years.
Cats with periodontal disease were much more likely to have a range of other health conditions compared to cats without periodontal disease (x 1.8 risk).
These conditions included cardiac dysrhythmia (x 2.3 risk), ear discharge (x 2.3 risk) and hairball/furball (x 2.3 risk).
The researchers say that these findings suggest that periodontal disease should be acknowledged as a leading health and welfare issue in cats, and highlight the need for greater dental care in cats as they age.
The study was supported by an award from the Kennel Club Charitable Trust and Agria Pet Insurance.
Bonqat 50 mg/ml oral solution for cats contains pregabalin, an analgesic, anti-epileptic which has now been licensed for the alleviation of acute anxiety and fear associated with transportation and veterinary visits in cats.
Tessie 0.3 mg/ml oral solution for dogs contains tasipimidine and is licensed for the short-term alleviation of situational anxiety and fear in dogs triggered by noise or owner departure.
Dômes Pharma points to research which shows that in the UK 1.4 million1 dogs show signs of distress when left alone, and 4.4 million dogs1 show fear of loud noises.
In addition, over 4 million cats2 are not having routine vet checkups each year, with 26%2 of these owners reporting that it is too stressful for their cat or themselves.

Dr Fures was convicted of driving with excess alcohol in the Dublin Criminal Courts of Justice in December 2018.
Later, when renewing his UK RCVS registration, Dr Fures told the RCVS Chief Investigator that on the day of his offence, he'd been on a flight from Frankfurt to Dublin which suffered engine failure, causing the pilot to perform a forced emergency landing in Amsterdam. There, he claimed, the passengers switched to an airworthy plane for the rest of the journey, during which he had several drinks to calm his nerves.
In a remarkably detailed and complex investigation, the RCVS Chief Investigator rang Lufthansa and discovered that the flight had not suffered engine failure and had flown direct from Frankfurt to Dublin without incident.
In May and July 2020, the RCVS Chief Investigator wrote to Dr Fures setting out the result of his investigations and research. In his responses Dr Fures accepted that his memory of the incident was wrong.
At the outset of the hearing Dr Fures made an application to the Committee enter into undertakings to voluntarily remove himself from the UK Register and to not apply to re-join. However, the Committee did not accept these undertakings in part on the basis that he was not of retirement age and intended to continue to practise in Ireland.
The Committee considered that if it were to accept his undertakings, then there would be no judgement or findings that could be passed on to the Veterinary Council of Ireland for consideration via its own disciplinary procedures.
Ian Arundale, chairing the Committee and speaking on its behalf, said: “The Committee decided that this is a case in which the public interest, confidence in the profession, and, potentially, the welfare of animals, demands that there be a full hearing, with determinations made by the Disciplinary Committee."
The Committee then went on to consider the facts of the case.
Dr Fures admitted that he had supplied the RCVS with false information about his conviction for drink driving but denied that this was dishonest at the time that he supplied the information. He accepted that the information he provided was wrong, in that his flight between Frankfurt and Dublin, while delayed by just over an hour, did not have to land in Amsterdam as he had previously claimed.
He said that his false statement was based on misremembering the circumstances and that he had genuinely believed his statement was true at the time it was made to the RCVS. He said that, due to shame over his conviction and the negative impact it had on the life of him and his family, he had created a false memory of the circumstances.
However, the Committee was not persuaded that there was any other explanation in this case, other than that Dr Fures deliberately and dishonestly gave false information to the RCVS, to excuse his behaviour.
The Committee then considered if the admitted and proven charges amounted to serious professional misconduct.
Ian said: “The Committee was of the view that Dr Fures’ actions in dishonestly giving false details to his regulator was serious. While it was the case that there was no actual harm or risk to animals arising out of his conduct, the Committee took into account that the dishonest account was given deliberately.
“In addition, it was sustained, in that it was relied upon and expanded upon on several occasions when the College sought further clarity. Dr Fures had the opportunity to correct the situation, and give the truthful account, but he did not do so. The dishonesty was designed to achieve personal gain to Dr Fures, in that he wished to minimise the actions which the College may take against him, and, in consequence, safeguard his career.
“Dr Fures’ action in dishonestly giving false information to his regulator struck at the heart of his obligation, as a registered professional, to be open and honest with his regulator. This obligation is necessary to allow the College, as regulator, to carry out its crucial and statutory functions in ensuring that it investigates concerns properly.”
In considering the sanction for Dr Fures, the Committee took into account the mitigating factors, including the fact that there were no previous regulatory findings against Dr Fures or any previous conviction for dishonesty, that he had demonstrated remorse for his actions, that there was no actual harm or risk of harm to any animal, that no concerns raised about Dr Fures’ practice, that there was no repetition of the dishonest conduct and that he had demonstrated some insight.
However, in terms of aggravating factors it considered that there was deliberate and sustained dishonesty and that he had sought personal gain as a result of his actions.
After considering various options, the Committee decided that a reprimand and warning as to future conduct was the most appropriate sanction for Dr Fures.

Membra is a transparent, water-resistant, non-toxic, biodegradable bandage replacement consisting of a hydrogel polymer which can be applied to open wounds.
It is initially applied as a liquid and quickly gels into a thin, flexible layer over the site.
The initial liquid phase of Membra allows it to cover open and irregular spaces as well as suture lines and it dries in minutes to form a barrier that covers and protects the incision site or wound.
Being transparent, Membra allows the wound to be seen by the veterinary team and pet owners.
Membra lasts for up to 14 days and naturally biodegrades over time.
Animus says that Membra lowers the risk of infection or dehiscence.
The company also says that hydrogel polymer has been shown to replace the need for adhesive bandaging on suture lines following major TPLO surgery.
A study concluded that the polymer did not appear to raise the prevalence of incisional complications after TPLO surgery and lasts for 14 days, rather than 24-48 hours with traditional bandaging1.

The 2,200sqm building – equivalent to half a hockey pitch or four netball courts – will include a range of facilities across two floors. It will have two new lecture theatres, a number of IT-rich teaching and learning spaces, general teaching rooms and various environments for teaching clinical skills.

Ian graduated from the University of Liverpool and gained his PhD from the University of Glasgow. He gained both the UK and European Diplomas in Small Animal Medicine at the University of Cambridge and is a Royal College of Veterinary Surgeons and European Recognised Specialist in Small Animal Medicine. He returned to Glasgow University in 1998 and has been the Professor of Small Animal Medicine since 2009.
Ian has published in the fields of infectious diseases, endocrinology and neurology but is interested in all aspects of small animal medicine. In 2015 he was awarded the BSAVA Woodrow Award for his contribution to small animal medicine. In 2016 he became of Fellow of the RCVS, thereby becoming a jolly good fellow Fellow of the RCVS, or JGFFRCVS for short.
Professor Ramsey has served BSAVA as a regional secretary, member of several committees and chair of Petsavers and Scientific. He is probably best known as the editor of four editions of the BSAVA Small Animal Formulary.
The Presidential handover usually takes place at BSAVA Congress but was delayed this year given the cancellation of the April event due to coronavirus.
Ian says that during his Presidency, he intends to promote and enhance the digital offering of the Association to BSAVA members coping with the pandemic. He will also oversee changes to BSAVA congress with a more interactive approach to training; shortened, more focussed lectures and greater integration between the exhibition, the scientific content, and the social events.
Ian said: “At this time of great uncertainty in the profession, my focus will be on providing BSAVA members with the scientific information and clinical training that they need, in a suitable form. Clinical skills and science lie at the heart of being a good vet and BSAVA members get access to a huge range of cheap or free resources in these areas to help them in their careers. Congress 2021 will be evolving as a vital part of this effort and I am looking forward to welcoming everyone to Manchester during the last week of March 2021."
