Zoetis has launched a 4ml vial of the antibiotic, Convenia.

Zoetis says Convenia gives veterinary surgeons control by providing an assured injectable treatment course, relieving clients of the burden of daily dosing. This avoids missed or mistimed doses, lost tablets, or courses stopped too soon, thereby eliminating a major reason for treatment failure, non-compliance. Poor compliance is also a significant factor in the development of antimicrobial resistance.

Convenia is licensed for skin, soft tissue and urinary tract infections in dogs and cats, as well as severe gingivitis/periodontitis in dogs (as an adjunct to mechanical or surgical periodontal therapy). A 4ml vial can treat up to 40kg of bodyweight, making it appropriate for treating cats and smaller dogs.

Zoetis says that the launch of the new vial demonstrates its commitment to the 'Respect Antibiotics' campaign launched last year. The Respect Antibiotics campaign encourages careful and informed selection of appropriate antibiotic therapies for the treatment of dogs and cats.

Following the outcry from the profession over the disciplinary hearing into Mr M Chikosi, the RCVS' new Operational Board has clarified the the College's position on the use of blankets to move animals.

The hearing found Munhuwepasi Chikosi guilty of unreasonably delaying attending a dog that had been run over at a farm, and of unnecessarily causing her to remain in pain and suffering for at least an hour.

As a result, the Disciplinary Committee directed that Mr Chikosi's name be removed from the Register for serious professional misconduct. The College says that since the appeal window has closed without an appeal being made, Mr Chikosi has now been struck off.

However, the Committee also said: "... his [Mr Chikosi's] advice that Mitzi should be moved on a blanket was wrong, as she may have had an injured back."

This was widely criticised as being out of touch with the practicalities of real life and unsupported by any evidence.

Speaking on behalf of the Board, President Neil Smith said: "We fully support the decision taken by the independent Disciplinary Committee with regard to the Chikosi hearing, with one comment requiring clarification: the issue of whether a blanket can be used to move an injured dog. We consider that it is acceptable, in most cases, to transport an injured dog with the aid of a blanket.

"The profession should be reassured that our Standards Committee [the new name for Advisory Committee] will consider the general issues raised by the Chikosi hearing at its next meeting. This will not be a review of the decision, but form part of the routine consideration of DC hearings made by the Committee to see if they raise issues that require additional guidance and advice."

The RCVS Disciplinary Committee has directed that the name of a veterinary surgeon who formerly practised in Norwich should be removed from the Register, having found him unfit to practise veterinary surgery following his Crown Court conviction for fraud.

During the one-day hearing, the Disciplinary Committee heard how Francisco da Cruz had abused his position whilst practising as a veterinary surgeon at Hellesdon Vets, his then workplace in Norwich, by defrauding a insurance companies of around £10,000 with fictitious claims for veterinary treatment on non-existent pets.

Following an investigation by the City of London Police's Insurance Fraud Enforcement Department (IFED), Mr da Cruz was convicted on five counts of fraud by false representation on 21 February 2013 at the Old Bailey in London, and later sentenced to eight months' imprisonment (suspended for two years) and 200 hours of unpaid community work; he was also ordered to pay just over £10,000 in compensation and costs.

Although Mr da Cruz had left the UK for Brazil shortly after his sentencing and was therefore not present at the hearing, the Committee was satisfied that he was deliberately evading the disciplinary proceedings, rather than being genuinely unable to participate in them, and so the hearing proceeded in his absence.

First accepting the copy certificate of conviction against Mr da Cruz as true, the Committee then had no hesitation in concluding that these convictions rendered him unfit to practise as a member of the veterinary profession. It found that the five counts of fraud were deliberate crimes of dishonesty, committed over a significant period of time and for significant financial gain. He had abused his position as a veterinary surgeon and abused the trust which the insurers placed in him as a professional.

Chairing and speaking on behalf of the Committee, Professor Peter Lees, said: "The Committee has no real confidence that there is no significant risk of repeat behaviour from the Respondent. His conduct subsequent to the criminal proceedings gives it no confidence that he has reformed himself to the extent that he will in the foreseeable future be fit to return to practice. So far from satisfactorily completing his criminal sentence, it appears that the Respondent has deliberately gone abroad to avoid doing so."

Bearing in mind that the purpose of any sanction it imposed was not to punish Mr da Cruz, but to maintain public confidence in the profession and uphold proper standards of conduct within it, the Committee concluded that the convictions were too serious to allow any sanction other than removal from the Register.

Zoetis, formerly Pfizer Animal Health, has launched Flexi4, a nutritional supplement for horses containing a patented combination of natural plant extracts designed to help maintain joint flexibility and the management of joint pain.

Flexi4 contains a blend of four plant extracts, Curcumin, Yucca schidigera, Salix alba and Boswellia serrata, which the company says have been shown to have anti-inflammatory, anti-pyretic and analgesic properties as follows:

• Curcumin, an extract from the roots of the herb Curcuma longa, has been shown to down-regulate inflammatory pathways and inhibit the production of inflammatory cytokines.¹ It has also been shown to reduce the release of GAGs from the cartilage matrix.²
• Yucca schidigera is a botanical extract containing saponins that suppress the intestinal protozoa, which are thought to have a role in joint inflammation in some circumstances.³ The extract also contains antioxidant polyphenols, which inhibit the enzymes of the arachidonic acid cascade, thereby reducing the production of inflammatory mediators.⁴
• Salix alba is an extract of white willow bark containing salicin, polyphenols and flavonoids. These are reported to have a positive effect on chrondrocyte viability, differentiation and function⁵ as well as anti-inflammatory, anti-pyretic and analgesic properties.⁶
• Boswellia serrata is the botanical extract of gum resin. Its role is to inhibit the pro-inflammatory cytokines and mediators which initiate the inflammatory process⁷ and produce anti-inflammatory activity at sites of chronic inflammation.⁸

An article by Consultant Biologist Clifford Warwick DipMedSci CBiol CSci EurProBiol FOCAE FSB in the current issue of the Journal of AWSELVA (the Animal Welfare Science, Ethics and Law Veterinary Association) is calling for greater scrutiny of the relationship between the veterinary profession and exotic pet trading.

The article, entitled 'Veterinary accountability and the exotic pet trade', was co-authored by biologist Catrina Steedman BSc(Hons) MSB and veterinary surgeon Emma Nicholas MA VetMB MRCVS

The authors accuse some vets of serving their own vested financial interests by directly aligning themselves with exotic pet trading. They say that these vets are effectively causing the wild-capture and intensive breeding, cramped storage, transport and sale of hundreds of thousands of fish, amphibians, reptiles and other animals.

The article points to scientific evidence showing that stress, disease and premature mortality commonly accompanies 'wild pets' at all stages of the trade and private keeping process.

The authors argue that promoting the trade in exotic animals runs counter to the veterinary profession's obligation to provide impartial guidance for the benefit of animal health and welfare, and that the issue of vets selling or promoting exotics in the home requires greater scrutiny for possible conflicts of interest and for contributing to harmful practices.

Lead author, Clifford Warwick DipMedSci CBiol CSci EurProBiol FOCAE FSB said: "By their nature, vets want to be and are obliged to be part of a solution to animal and human health and welfare matters. That a vet might have a hand in selling or promoting exotic pets is, in my view, tantamount to being part of a problem. And whether or not that represents a formal conflict of interest or questionable practice, I think the spotlight will continue focusing until there are no shaded areas."

He added: "In my view, the avoidance of unnecessary harm is a general responsibility we should all share, but I think veterinarians, like some other professionals, have a special responsibility in this regard. However, unnecessary harm is largely unavoidable with the exotic pet business. Whether wild-caught or captive-bred, animal suffering, species and environmental degradation, and public health concerns frequently go hand-in-hand with all stages of exotic pet trading and keeping practices."

Co-author Catrina Steedman BSc(Hons) MSB said: "Many vets are understandably critical of the exotic pet trade and its minefield of animal welfare, public health and environmental problems. It is unfortunate that a few vets, possibly with vested interests, feel it is appropriate to condone and even promote a trade that has such obvious negative consequences for animals and people."

Co-author, Emma Nicholas MA VetMB MRCVS said: "Vets have a responsibility to be approachable and impartial; animal welfare and also public health must be their priorities at all times. I believe it is pertinent that as a profession, we open the discussion as to how we best improve the welfare (health and quality of life) of existing exotic pets, and also educate and inform potential owners of "wild" or exotic animals of the associated welfare and public health considerations."

The article concludes that vets should '...occupy a detached position with no interest in the success or failure of the 'business' of exotic pet-keeping' and that 'any vet who recommends or endorses buying or keeping an exotic pet arguably imparts guidance that is not fully consistent with ensuring good animal welfare or human health'.

The RCVS Registered Veterinary Nurse Disciplinary Committee has suspended a Northants-based registered veterinary nurse who admitted to acting dishonestly with her employer, a client and a pet database company by taking home a patient that was supposed to have been euthanised.

During the two-day hearing, the Committee heard how Sally-Ann Roberts, formerly of the Best Friends Veterinary Group in Thrapston, had deliberately gone against the wishes of the owners of a 14-year-old Maine Coon cat called Jason that he be euthanised, rather than treated further, and instead had taken the cat home with her for "intensive nursing". Jason had subsequently escaped from Ms Robert's residence, leading her to fabricate a story, first to the pet database company, and then to Jason's owners and her employer, that he had escaped from the practice, before being returned by a member of the public two days later and then euthanised as originally requested.

Ms Roberts acted with her veterinary surgeon colleague Przemyslaw Bogdanowicz, who chose not to euthanise Jason and who, for his part, received a three-month suspension from the RCVS Disciplinary Committee in December 2012. She repeated the false account on a number of occasions, both orally and in written statements, and also forged the signature of Jason's owner on official documentation in order to substantiate her story.

Only when Ms Roberts was interviewed for a second time by her then employer's area manager, did she finally admit to what had actually happened. Shortly afterwards, Ms Roberts was suspended from the practice and, following an internal disciplinary hearing a few days later, was dismissed by them for gross misconduct, along with Mr Bogdanowicz. There was no evidence available as to what ultimately happened to Jason.

Explaining her actions to the Committee, Ms Roberts said she was upset that Jason's owners wanted him to be euthanised and felt that he could recover if given some love and attention. She had asked Mr Bogdanowicz to discuss this possibility with Jason's owners, but he had refused, agreeing instead that she could continue Jason's treatment at her home. After Jason escaped, Ms Roberts said she was "devastated" and had "panicked", inventing the story of Jason's escape to cover her actions, which she now acknowledged were "wrong" and "stupid", and which she "bitterly regretted". Ms Roberts expressed sorrow and remorse for her behaviour, which she said would never occur again, and stated that being a veterinary nurse was everything to her.

In view of the admitted facts, the Committee judged that Ms Robert's dishonesty and breach of client trust, as well the distinct risk of injury to which she exposed Jason, amounted to serious professional misconduct. In deciding on an appropriate sanction, the Committee balanced a number of aggravating factors (in particular, the forged signature) against Ms Roberts' "strong mitigation", which included her admitting the entirety of the charges against her, her medical and personal problems at the time, the insight she had shown into the effects of her actions on Jason's owners and her previous unblemished career.

Professor Peter Lees, chairing and speak on behalf of the Committee, said: "The Committee has concluded that the Respondent has shown insight into the seriousness of her misconduct and that there is no significant risk of repeat behaviour. In light of the Respondent's admission, her insight, her remorse and the high regard in which she is held by her professional colleagues, it is the Committee's view that the sanction of two months' suspension is appropriate and proportionate."

Maggie Fisher MRCVS, the parasitologist and chairman of ESCCAP (European Scientific Counsel Companion Animal Parasites), has highlighted studies which demonstrate an association between the common parasite, Toxocara spp. and human disease, and has called on the profession to help reduce the damaging and widespread effects of this parasite.

Maggie says that many in the profession may not be aware that Toxocara eggs aren't actually infective when they are first passed in an animal's stools, sometimes taking weeks to develop to this stage. Eggs persist long after pet waste has naturally washed away too, so the risk of infection still exists even in environments that appear to be clean and safe. Interestingly, there seems to be only a very loose (and according to some studies, entirely absent) link between owning a pet and an increased risk of human Toxocara infection, suggesting that most infections are acquired from the environment^1,2 . Education of pet owners about responsible pet ownership, including appropriate worming and picking up their dog's waste quickly is essential to reduce this environmental infection risk.

The parasite's larval stages cause disease in humans as they migrate through body tissues, with retinal damage a previously well identified consequence. However, Maggie has identified studies which suggest a whole range of common conditions that may also be attributable to Toxocara infection. One study³ included 425 children displaying signs of a persistent cough and 1600 asymptomatic children as controls. Blood tests confirmed that a statistically significant 32% of the symptomatic group were positive for Toxocara antibodies, compared to just 17% of the asymptomatic control group. What's more, when given a week's course of an appropriate anthelmintic, the vast majority of symptomatic children had a greatly reduced and frequently absent requirement for inhaled corticosteroids to control their symptoms when re-examined by the study team a year later.

Numerous other studies from around the world have shown similar strong associations with asthma, allergies and epilepsy. One nationally representative US study even showed an alarming association with Toxocara infection and reduced cognitive function in children² . Those that were positive for antibodies to the parasite on blood tests (proving prior exposure) scored significantly lower on tests assessing verbal and manual dexterity as well as maths and reading abilities² .

So, what should veterinary staff and SQPs be telling clients? Maggie says you should be advising pet owners to worm pets regularly (at least four times a year) with an effective anthelmintic from an appropriately young age (2 weeks of age for puppies and 3 weeks of age for kittens, because transfer of the parasite to puppies before birth and to kittens via milk is a feature of the parasite lifecycle). Reducing pets' access to raw food and hunting activities can also help. This is in addition to more general recommendations including cooking food well and making sure children wash their hands before eating.

Maggie said: "Vets, nurses and SQPs are perfectly placed to make sure that pet owners are aware of the risks and take precautions to protect themselves and the wider general public."

The RCVS has released the results of a survey which has found that increasing numbers of graduates over the last five years have had little impact on veterinary job prospects.

The survey was carried out for the RCVS by the Institute for Employment Studies, which asked the last five years' UK graduates who have registered with the College how long it took them to find work, how long they stayed in their first jobs, and why they moved on.

The online survey, which achieved a 43% response rate (1,354 responders), found that an average of 94% of graduates seeking a role in clinical practice obtained work within six months of starting to look. 

The actual figure ranged from a high of 96% in 2008 to a low of 92% in 2010, and did not change significantly over the five years under consideration, despite UK graduate numbers increasing by around a quarter during the same period (from 650 in 2007, to 819 in 2012). Meanwhile, the College has registered an average of 618 overseas graduates annually during this time. 

The survey did show that it was taking graduates slightly longer to secure their posts, with a shift from 85% securing work under three months in 2008, to 71% in 2012. 

The results seem to suggest some small differences in the time taken for men and women to find their first jobs, with men finding jobs slightly quicker, although the vast majority of both genders found veterinary work. 

Jacqui Molyneux, RCVS President said: "After the announcement from the University of Surrey that it will be opening a new vet school in the near future, there was a great deal of discussion amongst the profession about how easily new graduates could find employment. I undertook to get some real facts and am pleased to find that the picture is not as gloomy as predicted.

However, Jacqui said she was concerned that there has been a slight increase in the proportion of respondents who left their first position after a relatively short period of time. Amongst 2012 graduates, over 40% of those who had left their first position did so within three months of starting work. However it must be stressed that only 18% of those answering the survey who graduated in 2012 had already left their first position. Jacqui said: "Although the turn-over in first jobs seems to be, in part, due to an increase in temporary posts, I am saddened to see that the most commonly-cited reason for graduates leaving their first job was lack of support from their employers or professional colleagues.

"This is an area that we, as a profession, must address. As I have told all the students I have admitted to the College, their first jobs will influence their whole careers, and getting adequate support is probably the single most important factor. Meanwhile, it is heartening to see that nearly all of those moving on have obtained further employment."

Although the survey was sent to all those UK graduates who had registered with the RCVS within the last five years, the contact details for those who had subsequently de-registered may not have been up to date, which may mean that those who had de-registered because they could not find work were not well represented. However, the College says it thinks it is more likely that these individuals would have switched to the 'non-practising' category.

The British Equine Veterinary Association (BEVA) has released the results of a survey carried out earlier this year which has found that there may be up to 5 times as many veterinary graduates wanting to work in equine practice as there are jobs available.

The survey was carried out amongst the 2,500 vets and vet students who are members of BEVA to help ascertain manpower requirements of equine practices in the UK.

Of the 819 new veterinary graduates in the UK in 2012, BEVA says it estimates conservatively that between 10-15% intend to work in equine practice. Meanwhile, the extrapolated results of the survey suggest that in 2012 there was less than 1% growth in full time equine vet positions and as few as 24 new permanent jobs available in the UK.

BEVA says that in line with the survey's suggestion of restricted opportunities, some of its members are also finding that internships, commonly the domain of new graduates to gain paid, supervised experience in practice, are now often taken by vets with some post-graduate experience. Furthermore, unpaid externships, which traditionally gave vet students valuable work experience, appear to be being filled by unemployed veterinary graduates wanting to gain experience in order to improve their chances of securing a job in equine practice. 

Tim Mair, a partner at Bell Equine Veterinary Clinic in Kent said: "Earlier this year we had more than 80 applications for just one internship. Several years ago we would expect 50 or so applications for an advertised intern post but now, without advertising at all, we are constantly receiving applications, not just from new graduates but also from those who have been qualified for a year or more."

Charlie Schreiber, a partner at Donnington Grove Veterinary Surgery in Berkshire, said: "We take on an average of two externs a month and over the past four years have had more than 350 applicants but their calibre is noticeably changing. We used to hear primarily from students but now it tends to be recent graduates or those who have already done a job in mixed practice and are keen to move into equine but can't find an internship."

Andrew Harrison, a partner at Three Counties Equine Hospital in Gloucestershire, said: "In addition to the numerous applications we received for our recently advertised internship position, we are inundated with unsolicited enquiries about job opportunities from recent veterinary graduates from across the globe. Our most recent advertisement for an equine veterinary nurse has attracted interest from a handful of recently qualified vets desperate to find work. This employment problem is not confined to internships; some of our recent interns have found it very difficult, if not impossible to find a job in horse practice, once they've completed their internship."

Amanda Piggot, a recent graduate, has first hand experience of the challenge of finding a suitable placement. She said: "I have always wanted to work in equine practice and my approach has been very strategic. It took me five months to find my current position at Kessock Equine Vets in Inverness, which necessitated moving 500 miles further north than I ideally wanted. For my next position I applied to Liphook twice and had no luck but I have now been accepted by Rossdales for a two year internship."

According to the British Veterinary Association (BVA) the problem isn't confined to equine practice. The Association's President Peter Jones said: "The findings of the BEVA survey make sober reading and reflect the growing concerns in the wider veterinary profession about opportunities for new graduates in all types of veterinary practice.

"The issue has recently been brought into sharp focus by the announcement of a new veterinary school opening at the University of Surrey in 2014, with many BVA and BEVA members worried about an oversupply of veterinary graduates into an already competitive market. We are expecting the results of the RCVS survey of recent graduates shortly and, alongside the findings of the BEVA survey, they will provide an evidence-base to build on as we continue to assess the UK veterinary manpower implications of potential new veterinary schools in the future."

BEVA is keen to help both potential employees and employers tackle the situation as constructively as possible. BEVA President Keith Chandler said: "BEVA is proud that equine practice is attractive to so many younger members of the profession and it can provide a rewarding and fulfilling career. However we urge all stakeholders to ensure that current and prospective veterinary students are fully aware of the very limited opportunities in equine practice. Most of all we urge the students themselves to consider carefully their career path and prospects before making their final decisions."

Next month the RCVS will be releasing details of a survey of vets who have graduated within the past five years and how easy they have found it to find a job that meets their needs.

The RCVS has responded to the outcry sparked by the Disciplinary Committee striking off a veterinary surgeon that delayed an out-of-hours home visit to a dog that had been run over by its owner, a farmer.

The RCVS response addresses three main issues raised by the case:

• Many have expressed concern that this rule sets some sort of precedent through which any veterinary surgeon who doesn't attend a home visit within a certain time frame will automatically be struck off. The RCVS says that it has no legal framework for taking precedence into account.
• Some of the comments made in the case summary appeared to demonstrate a lack of veterinary knowledge on the part of the DC, leading to questions over whether lay members in particular have any training or experience of the issues facing veterinary surgeons in practice every day. In the College's response it highlights that DC members undergo one-day induction training, two-day annual training and any additional training as and when necessary. However this is legal training and lay members do not have to have any experience of veterinary practice.
• There has been some discussion concerning whether the ruling was in keeping with current advice in the Code of Professional Conduct, or whether it represented a new commitment for veterinary surgeons to attend home visits (where necessary) within the hour. The College confirmed that the current advice in the Code on 24-hour first aid and pain relief is consistent with the decision:
  • 1.4 Veterinary surgeons in practice must take steps to provide 24-hour emergency first aid and pain relief to animals according to their skills and the specific situation.
  • Supporting guidance can be found at www.rcvs.org.uk/247care, and includes the following: 3.14 Clients may request attendance on a sick or injured animal away from the practice premises and, in some circumstances, it may be desirable to do so. On rare occasions, it may be necessary on clinical or welfare grounds. The decision to attend away from the practice is for the veterinary surgeon, having carefully balanced the needs of the animal against the safety implications of making the visit; a veterinary surgeon is not expected to risk 'life or limb', or that of anyone else to provide the service.

However, the College has not yet responded to many of the other issues being debated, including:

• The case report says: "The Committee is satisfied that the delay caused Mitzi [the dog] unnecessary suffering, which was evidenced by her uncharacteristically biting Mr Matthews' [the owner] son." What evidence does the DC have to support this assertion that uncharacteristic biting is evidence of unnecessary suffering? 
• The case report says that the defendant: "...offered no advice to Mr Matthews as to how Mitzi's condition could be alleviated whilst waiting for the home visit." That beggars the question what advice would it have been possible to give under the circumstances (i.e. an animal that has been run over and not yet seen by the veterinary surgeon)?
• The case report says: "His [the vet's] advice that the dog should be moved on a blanket was completely wrong, as the dog may have had an injured back." What evidence base does the DC have to support this statement? How else would the RCVS recommend an animal is moved?
• The case report says: "The Committee listened carefully to the record of the four telephone conversations, which it considers showed an absence of compassion on his [the vet's] part." Was this merely a result of a cultural difference? If Mr Chikosi sounded not compassionate, but businesslike, would that not have been equally appropriate for an emergency situation? Should veterinary surgeons now fear that if their bedside manner is not up to scratch, they face removal from the register? Is it appropriate for the DC to comment on a veterinary surgeon's manner, or is it more appropriate to allow market forces to apply? 
• The severity of the sanction. Striking off is the ultimate penalty; the removal of a veterinary surgeon's ability to earn a livelihood in the UK. Mr Chikosi made one (arguably) poor decision which led to a delay in euthanasia of an hour. Should he have faced the ultimate sanction for this? In a poll on vetsurgeon.org, 53.7% of 123 participants felt Mr Chikosi had no case to answer in the first place, 40.7% said he acted unprofessionally, but the sanction was unduly harsh. Only 5.7% said the sanction was appropriate.
• Is it fair that the ultimate sanction can be applied in situations where the respondent has failed (for whatever reason) to respond to the charges and is not represented at the hearing? The case report says that documents were emailed and posted to Mr Chikosi in Zimbabwe, to where he had returned, but not whether receipt was acknowledged.
• Where in the ruling was there any recognition of the owner's responsibility to the animal and its welfare? Should a farmer and his son reasonably have been able to transport the animal to the practice?
• The case report says that: "Mr Matthews [the owner] told him [Mr Chikosi] that it would only take Mr Chikosi ten minutes to come out to the farm. Was this, in fact, more a turn of phrase than a fair reflection of the likely journey time? If not a turn of phrase, is it likely an underestimate (because the owner was trying to persuade Mr Chikosi to come), particularly if Mr Chikosi was unfamiliar with the address. Either way, many have commented that with the return journey, the likely time away from the practice would be at least an hour, which would further explain Mr Chikosi's reluctance to leave the practice and attend the house.  
• Exactly what responsibility does Mr Chikosi's employer bear for the fact that he was in a situation whereby he felt unable to do a home visit?  Varying reports suggest a difference in Mr Chikosi's employer's official written policy towards home visits, and an unwritten policy. Alternatively, he may simply have misunderstood the policy, written or otherwise. Was it explained clearly?
• Is it fair to enforce the harshest penalty on a veterinary surgeon who carries out orders which go against the GtPC when they might face the loss of their job if they do otherwise?
