The study was based on VetCompass anonymised clinical records from more than two million dogs under first opinion veterinary care in the UK in 2019.
The study applied causal inference methods to the analysis of random samples of 815 dogs with CCL rupture aged between 1.5- and 12-years-old to replicate a randomised clinical trial and explore the impact of the different treatment methods.
Balancing the arms of the study for variables including age, breed and bodyweight, the researchers compared the outcomes for the dogs who received surgical management for CCL ruptures to those that were managed non-surgically.
The results showed that dogs managed surgically were 25.7% less likely to show short-term lameness at 3 months post-diagnosis and 31.7% less likely to show long-term lameness at 12 months post-diagnosis than dogs managed non-surgically.
Surgically managed dogs were 38.9% less likely to have a pain relief prescription at 3 months post-diagnosis, 34.1% less likely at 6 months and 32.7% less likely at 12 months than dogs managed non-surgically.
The RVC says these findings also similarly demonstrate lower longer term pain in surgically managed dogs.
Camilla Pegram, VetCompass PhD student at the RVC and lead author of the paper, said: “This study used an exciting new approach that allowed us to determine ‘cause’ rather than being limited to ‘association’.
"Surgical management for CCL rupture is often considered as providing better outcomes than non-surgical management, but this study now provides an evidence base to support this.
"Whilst surgical management might not always be feasible for some dogs, the findings allow vets to quantify the benefit in their discussions with owners.”

They say that dogs which lose sensation following damage to the spinal cord from a slipped disc have a prognosis for recovery of about 50%.
However, there is no reliable way of determining which dog will recover and failing that, owners must wait weeks to months to see if there are any signs of recovery, during which time dogs may need anaesthesia, surgery, and intensive nursing care.
The researchers say that the stiffness of an organ can be a marker of how damaged it is.
So, for the research, a new ultrasound machine equipped with an advanced piece of software (known as ‘shear wave elastography’) – will be used to measure the stiffness of a dog’s spinal cord during surgery.
The dog’s recovery to walking after surgery will then be monitored routinely.
The results will allow researchers to see if a relationship between spinal cord stiffness at time of surgery and recovery exists.
If successful, this would help them provide a more accurate prognosis for dogs with spinal cord injury in the future.
If you have a case suitable for this study, CVS says they will receive a gold standard treatment for spinal cord injury, which will include spinal surgery.
They will also have an ultrasound of the spinal cord performed during surgery, a procedure taking approximately 15 minutes.

The Veterinary Nursing foundation degree at Aberystwyth University combines theoretical and practical learning, with a year-long work placement in a veterinary practice as an integral part of the programme.
Facilities at the university include a clinical skills lab, research and anatomy laboratories, equine teaching centre and university farms.
A mock veterinary clinic is being developed and will allow students to prepare for clinical placements in a simulated environment.
The School of Veterinary Science at Aberystwyth University has been training veterinary students since it was officially opened in December 2021 by King Charles III.

The group says it has led to 6,086 more clinical metrology questionnaires being conducted at participating practices with their patients, a 1,014% increase.
Developed by John Innes at the University of Liverpool, the Liverpool Osteoarthritis in Dogs (LOAD) questionnaire is a 13-item clinical metrology instrument (CMI) used to assess canine articular disorders like osteoarthritis.
It includes questions on the pets’ medical background, lifestyle and mobility.
Individual question scores are summed to provide an overall “LOAD score” suggestive of the animal’s disease presence and severity.
The questionnaire should be conducted every time the pet is presented to the veterinary clinic, with results tracked over time.
Treatment options - including the benefits and risks of all available modalities – can then be considered and discussed with owners to determine the best course of action, including weight management, diet, supplements, exercise and environmental modification, in addition to medication.
As part of the project, CVS provided a range of supporting resources for its practices, together with training.
Training focussed on diagnosing and managing conditions causing lameness in dogs and cats, using the clinical metrology questionnaire to engage clients in their pets’ treatment plan, offering best practice treatment options and running nurse mobility clinics.
It comprised of a vet CPD day run with Zoetis and the VOA on approach to and management of lame patients, two nurse CPD days on mobility clinics, a feline and canine orthopaedic examination video, First Opinion Clinical Club webinars on canine and feline osteoarthritis and an approach to radiographic interpretation course.
Resources included: new clinical guidelines, IMV resources (radiographic positioning, optimising the image and optimal chemical restraint) and a Mobility Clinic Booklet for clients.
Each site was then provided with its own data, so that they could monitor what they were doing individually.
One year after its nation-wide launch, results reveal that the number of clinical metrology questionnaires being conducted in the 35 participating practices has increased from 6 to 6,092 over a 12 month time period.
CVS says there was also evidence of an increasing incidence of the osteoarthritis drugs Librela and Solensia being prescribed.
Kathryn Patel, Hub Clinical Lead at CVS (pictured), who was responsible for the lameness clinical improvement project, said: “This project uses owner questionnaires to screen for and monitor the impact of lameness on patient quality of life and emphasises the importance of the whole clinical team in patient management.
"Vets and nurses will be supported to improve their ability to reach a diagnosis in every lame case, improve engagement with and compliance of owners with lame pets, and improve the quality of care of lame patients through appropriate medication, surgical intervention, and nurse and physiotherapy support.”

Mr Seymour-Hamilton made previous unsuccessful applications for restoration in 1995, 2010, 2015, 2016, 2017, 2018, 2019 and 2020.
Mr Seymour-Hamilton was originally removed from the Register in 1994 following an inspection of his veterinary practice premises in Orpington, Kent, that found that the condition of the practice, including the equipment and facilities, were of such a risk to animal health and welfare that it brought the profession into disrepute.
In considering his current application, the Committee had to take several factors into account, including: whether Mr Seymour-Hamilton had accepted the findings of his original hearing; whether he demonstrated insight into his past conduct; protection of the public and the public interest; the future welfare of animals committed to his care should he be restored; length of time off the Register; his conduct since removal; and evidence that he’d kept up-to-date in terms of the knowledge and skills required of a practising veterinary surgeon.
Mr Seymour-Hamilton accepted the facts of the original charges but did not accept the conclusions of the two veterinary surgeons who inspected his practice, who, he claimed, had falsely accused him.
As a result, the Committee concluded that Mr Seymour-Hamilton did not accept the seriousness of the original findings against him, nor had he demonstrated any insight into either the original charges, nor what was required of him to enable a successful application for restoration.
Judith Webb, chairing the Committee and speaking on its behalf, said: “The Committee is driven to the conclusion that the applicant has displayed total disregard for rules and regulations. In his evidence he stated that he is professionally competent to spay a cat and he had done so in Calais. This was in recent years and whilst struck off by the RCVS, and after more than 25 years out of the profession. He was unable to see that he had done anything wrong in this.”
As to protection of the public and the future welfare of animals committed to his care, the Committee found that Mr Seymour-Hamilton continued to display a lack of understanding of the role and purpose of regulation. He had shown no insight into previous decisions or of what was required of him to enable a successful restoration, despite being given advice in that regard. He presented no compelling evidence to indicate that he would be safe to practise veterinary surgery were he to be restored to the Register.
Regarding Mr Seymour-Hamilton’s conduct since removal from the Register, the Committee found he did not demonstrate sufficient relevant conduct in relation to his fitness to practise. The Committee noted that, on previous occasions, he had indicated he had illegally practised veterinary surgery and admitted using his own animals to experimentally administer new and untested remedies.
The Committee found that, in relation to the 27 years since removal from the Register, Mr Seymour-Hamilton had not undertaken the prolonged, intensive and formal retraining needed to ensure he was now fit to practise. It also found that, since the last restoration hearing in 2020, Mr Seymour-Hamilton had undertaken very little continuing professional development, maintaining that he was too busy with his work relating to herbal remedies to attend formal veterinary training.
Judith Webb added: “He claimed that he does not find it difficult to keep up-to-date, because he is able to perform online searches if he is in need of information. He thinks that he is competent to operate even after 27 years out of the profession, and maintains that he could operate now. In the view of the Committee, the applicant is totally unaware of current veterinary principles such as it being an evidence-based profession, use of clinical audit and reflective learning. He does not appear to have accepted the purpose of the regulator in protecting public interest, including maintaining public confidence in the profession, nor its role in upholding professional standards and promoting animal welfare.
The Committee considers that where some 27 years have passed since the applicant has practised, there will inevitably be a serious risk to the welfare of animals if he is restored to the Register. In addition, the Committee is firmly of the opinion that it would not be in the public interest for the application for restoration to the RCVS Register to be granted in this case.”

Tip 1 – Make sure that pet owners understand the functions of a microchip properlyMichelle says: "Some customers I’ve spoken to wrongly believe that microchips function as GPS trackers. To help manage pet owners’ expectations I always stress the importance of a microchip in lost pet recovery scenarios, where they serve as a permanent form of identification."
Tip 2 – Stress the importance of pet owners updating their contact details with their microchip providerMichelle says: "Pet owners often think that updating contact details with their vet will automatically sync these changes with the microchip database. It’s vital to inform owners that they must separately update their contact information with their microchip database. I’d recommend regularly reminding pet owners to check and update their microchip registration."
Tip 3 – Reassure pet owners that microchips are safe for their catsMichelle says: "I’ve had some owners express worries about allergic reactions or rejection of the microchip by their cat's body. So it’s worth taking the opportunity to dispel this myth during routine wellness exams by explaining microchips are made from inert, biocompatible materials designed to be nonallergenic and nontoxic.
Tip 4 – Make sure to discuss the longevity of microchips with pet ownersMichelle says: Microchips are designed to last the lifetime of the pet without needing a power source and that their functionality can easily be verified during any veterinary visit. If you find an owner expresses concerns about this, you could offer to check the microchip as part of regular health checks to demonstrate its reliability.
Tip 5 – Emphasise how every cat needs a microchip – regardless of age or activity levelMichelle says: "All vets will know even indoor or elderly cats can escape or wander off and older cats can become disoriented more easily. It is worth reminding owners that microchipping is a safeguard, ensuring their pet can be identified and returned home if lost. The new law means that pet owners can also face a large £500 fine if their cat isn’t microchipped."
Tip 6 – Explain how not all microchips are the same and the importance of getting the right oneMichelle says: I often explain that microchips vary in terms of the technology they employ, such as the new 'Thermochip' by HomeAgain that also measures a cat’s subcutaneous temperature. It’s worth talking pet owners through the different microchips available and the database options so they can make an informed choice.

Historically, diagnosis has been achieved with multiple blood tests. However, this process can be time-consuming, expensive and in some cases does not provide accurate results.
In an attempt to address these diagnostic challenges, the researchers, with funding from Dechra, assessed anonymised data from 939 dogs which had been tested for Cushing’s syndrome from the VetCompass population of 905,554 dogs and 886 veterinary practices across the UK1.
Using structured clinical data to look at the dogs’ demographics, clinical signs at presentation and laboratory results, machine-learning algorithms were applied to predict a future diagnosis of Cushing’s syndrome. Dogs suspected of having Cushing’s syndrome were included in the analysis and classified based on their final reported diagnosis within their clinical records.
The researchers say that the findings indicate that machine-learning aided diagnosis could predict the diagnosis of a practising veterinary surgeon and that using machine-learning methods in clinical practice could contribute to improved diagnosis of Cushing’s syndrome in dogs.
Additionally, further development of these algorithms could lead to earlier, more reliable and cost-effective diagnoses and therefore, better clinical care for dogs with Cushing’s syndrome. This could also create opportunities for this technology to be applied to other clinical problems.
Imogen Schofield, lead author and PhD student at the RVC, said: “Machine-learning algorithms, like those used in this study, are already widely integrated in our day-to-day lives to help make certain decisions, such as Google or Netflix recommendations. Now this technology can be harnessed to help improve diagnostics in veterinary practice.
"By embracing the use of machine-learning methods, we are a step closer to providing vets in primary-care practice with an easy to use, low cost and accurate test that can support the often frustrating process of diagnosing Cushing’s syndrome in dogs."
Greg Williams, Senior Business Manager at Dechra Ltd. and Industrial Supervisor of the PhD studentship, said: “By funding Imogen's PhD and working with the RVC we have been able to develop validated clinical scoring and quality-of-life assessments to help vets deliver effective control and management of Cushing's syndrome in dogs.
They include the successful completion of its governance review, the launch of the Graduate Outcomes consultation (the biggest in 20 years) and the Edward Jenner Leadership Programme (a massive open online course to develop leadership skills at all levels of the profession).
The College also highlights the continuation of its Mind Matters Initiative and how the initiative's aim - to encourage a compassionate and empathetic profession - is becoming a central part of the College's core strategy.
The report also covers the work done by the College to explore an outcomes-based approach to continuing professional development (CPD), review Schedule 3 and the Veterinary Surgeons Act, plan for the UK’s exit from the European Union and promote (jointly with the BVA) the benefit of registering with a vet to the public via a social media campaign.
The report includes the College’s independently-audited finances with details of income and expenditure. Whilst the College is not a charity, the accounts have been prepared in accordance with the Charities Statement of Recommended Practice – a framework for charity accounting and reporting, which allows easier comparison with the finances of similar bodies.
The report will be presented for adoption by members of the College at this year’s RCVS Day on Friday 12 July 2019 at the Royal Institute of British Architects.

Under the new policy, introduced this month, women who joined the company more than two years ago are entitled to six month's maternity leave on full pay, while those who have been with the company for less than that are entitled to three months at full pay and three months at half pay.
After six months, statutory rates will be paid.
Meanwhile, new fathers are now entitled to four week's paternity leave on full pay.

SRUC, which is the biggest provider of veterinary nursing, livestock husbandry and animal care training in Scotland, says it will offer a core veterinary programme to address existing shortages in veterinary provision, in areas such as rural veterinary practice, food production, food safety and animal and public health.
The College is first establishing a working group to progress the plans for the school. The group will be chaired by Professor Sir Pete Downes, former Principal and Vice-Chancellor of the University of Dundee.
Sir Pete will be joined by Sheila Voas, Chief Veterinary Officer for Scotland, former NFUS President Nigel Miller, SRUC Board member Jane Craigie, and Dr Kate Richards, who is a non-executive director on the SAC Commercial Board and currently the junior vice president of the RCVS, in line to become President in July.
Professor Caroline Argo, currently Dean of SRUC’s North Faculty, will lead the project for SRUC.
A report from BiGGAR Economics has found that the vet school could add £26 million GVA and 242 jobs to Scotland by 2030.

The Diabetic Clinical Score is a quick, standardised scoring system to record the clinical signs of diabetes in a patient, which was designed by Royal Veterinary College Internal Medicine Specialist Dr. Stijn Niessen.
The scoring system has been turned into a pet diabetes app which allows clients to record and monitor the impact of treatment.
CVS' aim is to use the recently validated tool in over 80% of patient cases to improve the treatment and control of diabetes whilst supporting the practice team to develop their skills in managing the condition.
Participating practices will use the DCS, alongside blood glucose measurements, to diagnose and monitor patients.
Veterinary nurses will refer animals coming in for six-monthly Healthy Pet Club check-ups to a vet for a possible diabetes diagnosis, whilst vets diagnosing diabetes will introduce owners and patients to the DCS to record a diabetes log, to help manage the condition.
To upskill and develop colleagues in the use of the DCS, dedicated vet and nurse training sessions have been held by the clinical leadership team, including a kick-off ‘diabetes day’ which focussed on current thinking and introduced the DCS tool.
CVS teams have also grouped together to agree a coordinated approach to managing pets with diabetes in their practice.
Supporting diabetes resources have also been drafted for practices, including new clinical guidelines, supporting paperwork and CPD.

“The Diabetic Clinical Score will help the whole team around the patient – the owner, vet and veterinary nurse – to effectively and cohesively manage their care.
It will implement joined-up thinking and help to put a standardised disease management framework in place.
“It will be fantastic to see the benefits that the Diabetic Clinical Score will bring to our patients and practices.
"I am certain that this will help contribute towards them providing some of the best diabetes clinical diagnosis and care in our profession.”

The training materials include downloadable teaching guides and two videos fronted by Dr Russell Chandler BVSc CertSAO MSc(OrthoEng) MRCVS, an Advanced Practitioner in Small Animal Orthopaedics (pictured).
The videos are designed to help GP vets become more confident about performing intra-articular injections in the hip and elbow, sharing principles and techniques which are also useful for performing arthrocentesis.
Specifically, the videos show the precise procedures, in both elbow and hip, for accessing joint spaces, confirming accurate placement by the appearance of synovial fluid and injecting DogStem.
The training guides offer supporting diagrams and explanations of the approaches to each joint.
