Again by popular demand, Fiona Lord VN has made her nursing care plan available for all to download here.

She explains: "On the front of the plan is an admission form. On the reverse is the plan itself. It's designed to help assess each patient as an individual and set out a plan of care, with goals. There is space to record any possible problems that arise, and nursing actions taken. The plan for each patient should be assessed on a regular basis, depending on the individual, and then altered accordingly."
