Absolutely 100% agree with you, and it's kind of my point ... which is that if the profession gets all tied up in knots over buzzwords, the danger is both that the public doesn't understand them and also that the profession loses focus, which should be on exactly what you say: making sure owners understand about the options and costs.

And I would add ... the profession having a greater focus on keeping costs down (for example by recommending treatment that is more financially in keeping with the species being treated).
If the public also received the same from the NHS, a 'what it would have cost you', they would know what the 'hidden' costs were there too and be able to appreciate and understand more - I would hope anyway.
I agree, people should have a much better understanding of the value of what they get from the NHS.

Film Club: Are you a pragmatic vet nurse?

With the cost of veterinary care going up so much in recent years, not to mention the Competition and Markets Authority investigation of the profession, two buzzwords have emerged: "pragmatic" and "contextual" veterinary care.

Some people think pragmatism suggests a lower standard of care. So I went to go and talk to the Blue Cross about it, and see how they define pragmatism, after all, they need a lot of pragmatism in the charity sector.

Here's what they said. What do you think? Are you a pragmatist or do you lean more towards idealism?

Would be really interested to hear the nursing perspective on this ...

Damn damn damn and triple damn!
Great idea.
But thanks!

Rachel Woodcock - one other thought, though ...

I am thinking I'd love to have contributions from around the UK, but cant realistically fly up to the Shetlands to film a 1 minute film showing how to tie a suture. So I would like to encourage everyone to submit their own demo of how to do something. I just thought it might be a good idea if I start by demonstrating the sort of thing that could work, and the standard I'm looking for (which is basically good enough that you can see and understand what is being demonstrated).

If you or anyone else is happy making a short film on a mobile phone and sending it to me, we could also do that.

Perhaps demonstrate any of these: https://cdn.harper-adams.ac.uk/document/page/982_OSCE-Task-Booklet-202525.pdf

I'd love it if we could get as many nurses as possible to chip in and demonstrate how to do something. It would be such a lovely resource. All in it together.

Can anyone help me with this idea?

I wonder if anyone can help me ...

I have this idea of creating a bank of short 'Show Me' films in which RVNs demonstrate how to perform a nursing task.

They would all then be kept in a searchable resource here and on YouTube, for the benefit of others in the profession, especially students and newcomers and future entrants.

I envisage these films being quick and short, and for some tasks there may be more than one demo as different people have different ways of doing things.

I think this would be a wonderful new resource and a lovely way for everyone to help each other.

A starting point could be OSCE tasks.

Would anyone like to help me get this going by allowing me to film them demonstrating a veterinary task?

Ideally I would film in practice and it takes a very short time. The practice might also be pleased because the films can be branded with the practice logo and used as a recruitment tool (if you are recruiting).

I'm based in the South West, so to start with, I'm ideally looking for people within an hour of Bath, or in London.

What do you think? Good idea (or not)? Can you help?

RE: Can anyone help me with this idea?

Great idea.
pragmatic and contextual as being part and parcel of the same decision making process
I come at this from the perspective of being a communicator (ie journalist / PR) - and I fear that these terms just don't address the real elephant in the room.

The problem is that veterinary care has become too expensive. Who says so? After all, what is wrong with saying pets should be exclusively for rich people who can afford them?

I think we can agree that they bring so many social benefits that there is a strong argument for care remaining affordable.

Both pragmatic and contextual advice are things which vets and nurses have ALWAYS had to offer, but they have taken on a special importance since the CMA investigation and the public reaction to rising costs.

They seem to me to be woolly concepts which have no universal definition. They mean different things to different people. And they don't focus the mind squarely on the real issue here: COST.

So whilst pragmatism and contextualism are both important, for the purposes of responding to the pricing issue, I think we need to be talking about money and cost directly.

RE: Film Club: Are you a pragmatic vet nurse?

This is a really good read Maximising welfare benefits by contextualising case management - RCVS Knowledge I see pragmatic and contextual as being part and parcel of the same decision making process: sensible, satisfactory 'good enough' and then offering more options with a utilitarianist, consequentialism perspective, fully exploring what is best

Film Club: Our first RVN star ...

Just published the latest in our series of short films about pragmatic veterinary care, in which the lovely people at Blue Cross share their approach to pragmatic vet medicine.

In the latest one, Charlotte Beard RVN talks about the charity's approach to pragmatic veterinary diagnostic testing.

She was a star - I don't think she'd done anything like this before, and had no preparation. Just jumped in front of the camera, and I think she did a superb job.

What do you think?

And what do you think about diagnostics in practice. Do you think there is overuse of diagnostic testing? Or not.

And whilst I'm here, I wonder also whether there might be any other potential stars who would like to star in a short film demonstrating veterinary nursing tasks (prob OSCEs) for the benefit of everyone in the profession, but especially students and future generations!
