Otherwise that should be fine. The wording of the K&amp;amp;U makes it sound like they are looking for something more, but to be honest that should be sufficient.&lt;/p&gt;

&lt;p&gt;for K&amp;amp;U j,&amp;nbsp;you just need to explain how you would write it up....but perhaps cross referencing it to a caselog where you have included a copy of a sheet would be sufficient. Otherwise to be on&amp;nbsp;the safe side, explain the necessity for the case details, perhaps case vet, dates, all the details of the meds required, and the need to mark off each as it is done with persons initials to ensure accountability and stop any errors in dosing.&lt;/p&gt;

Highlight the meds you gave on a photocopy of the form, and use it as additional evidence to support the caselog or DOA you have to evidence the criteria.&lt;/p&gt;
