Hill's weight display has these details on as well as in their little weight clinic info leaflets.

We used to have a disc from hills with this on but we have "misplaced" it and so have asked our rep to find one for us but apparently they have stopped making them!

Royal Canin do/did them in their little weight management packs for clients. I think they did them in A4 poster form too and were very useful at getting the information through!

We have one made by a previous vet nurse, but I think the info came from a sheet from Hills - maybe worth giving them a call?
