Where I used to work we made up our own, which I like taylor made to include everything we wanted. Now where I work part time we use the ones from AAS

Correct me if i'm wrong, but they're the gold star ones aren't they? We use them, didn't like them to start with but personally i now really like them, the amount of info you can put on them is great. We used to have home made ones but ditched them!

we use the sheets from Intervet Schering-Plough Animal Health, I think we might have had 1 pad with sheets once but we use copies now.
I found them quite easy to start with.

do you have a fax number?? I can fax you one when I get to work on monday .. our monitoring forms are easy to read and jot down recordings
