To the best of my knowledge there is no hard and fast rule when there are two (or more) addresses and no further info. I would just pick one of the addresses, pick the relevant town/county and state that. I have had to do that in the past when it says X publisher and the lists them as in New York, Toronto and New Delhi (or whatever). Has not caused me a problem to do this in 7 years of referencing assignments, etc and it happens all the time. If you find out different though for definite I would be interested to know!

hmm.. no, one just says printed in USA and gives two addresses for the publisher unfortunately uni guidelines are offline at the mo.. will have to probe a librarian on monday maybe...

Does it state the name of the printing firm? Is this the same as the publisher? Sometimes that's why and the addresses are interchangeable.

Some books do not seem to state the place of publishing, just where printed

Books are listed by author, date, title, place of publication, publisher:
