We call them release of ownership,

Just type them up with the owners name and address and the animal to be signed over.

'I hereby release ownership of the above animal to veterinary practice name and address'

Then the owner signs it and someone on behalf of the practice with the date

i wrote my own on both occasions i have taken dogs otherwise we have rspca ones.

When this happened not so long ago the bossman just printed a consent form out, instead of treatment he just wrote about signing it over for adoption
