oh dear Vicky lololol

When trying to internally empty, put your finger in to the anus at four o'clock and eight o;clock. If they are full/impacted, you will feel pea sized lumps - just squeeze gradually, use gloves and cotton wool and you will feel them emptying....................

I was taught to empty externally but never found i could get it to work very well unless they were very full. My HN taught me how to do external cos that's how she does it but i asked the vet to show me internal way because i want to be sure i've emptied it all out. The other day i just went to feel Jacks externally to see if they needed emptying...must have been very full cos they started to empty onto my hand. Because i wasn't planning on emptying them i wasn't wearing gloves

yup you have hit the nail on the head but if an owner comes in with a dragging bum I suppose that would be your only indication or if they were excessively full you can sometimes feel them as a couple of 'peas' . Have seen anal glands burst when they have been done this way (very infected anal gland , and thankfully it was a vet rather than a nurse) - which is why I dont like doing them like this

That's so true. For something like nurse clinics if they're run with the rest of the team backing you up they can be a real asset to the practice, as well as increasing nursing morale (only of course if the nurse enjoys doing them, and isn't being made to!) Wish more practices would encourage them.

A little tip if you have to do it this way get who ever is holding the dog to pull forwards very slightly with the front end , usually the dog naturally resists this and pulls back straight into the hands of the person doing the deed - it can make things so much easier.

it sure sounds like it works, the only problem I would have is that you wouldnt actually know if the glands needed emptying in the first place and if you actually got anything out of them

Yep! My last practice were not going to offer a pay rise to QVNs, yet had increased vets/receptionists/SVNs salaries. As I was the only QVN at our branch I set up a number of clinics, worked out how much income I'd generated to the practice over six months and presented this to the partners. The amount was on par with a vet who had just been there for those six months so was pretty pleased about it. Got the pay rise too!

Some vets/nurses do it by just squeezing either side of the anus with finger and thumb... that's how I was taught to do it originally... I have since learnt how to do things better and with less discomfort ... even tho I still am not 100% confident about doing it!

I really ued to dislike doing nure clinics but I think was becaue I was never reall taught what to do!

Now I love them I can spend half an hour with a client and the clients love it. And I think the clients feel like they havnt wasted any body time for coming in for advice, which I think they would if they had seen a vet, We do anal gland, charge £15 which I think its a bit steep but we have a groomer in house who doe most of them routinely so we dont tend to get a lot booked in.

I like to do them internally too so much easier!

As for hearts etc I always liten as I think it good just to get to hear the norm but I always dread the client asking if it alright alway worried of aying sound normal and hen coming back later with heart failure xx

nice one! - once I had proved to the practice the nurse clinics were actually a really good source of revenue as well as freeing up some of the vet consult time (and were not in any way a threat to vet clinics as we dont diagnose ) I never really got any opposition
