Our vaccine folders even have a page for them to fill out before the booster appt, but no-one ever does!&lt;/p&gt;
&lt;p&gt;A one-sided form to fill out while they wait in reception seems like a good idea, as lots of clients (I think anyway!) forget things, especially once the consultation gets going. Plus, it will keep them busy while they wait for the vet.&lt;/p&gt;
