&lt;p&gt;ive had one cancellation recently after 4/5 years of locum life and it did make me pause and consider adding in a cancellation fee to my contract if within a certain time frame. i havent added it yet though.&lt;/p&gt;
&lt;p&gt;i dont charge mileage for practices but if they are further away from home/need last minute cover i do charge more than my &amp;#39;usual&amp;#39; rate. (i dont usually do this for my regulars though)&amp;nbsp;&lt;/p&gt;
