&lt;p&gt;Merck Vet Manual has a section with an evaluation section &lt;em&gt;&amp;#39;Parameters to Evaluate During Triage&amp;#39;&lt;/em&gt;. Might help you to scavenge that and prepare a format you&amp;#39;re happy with?&lt;/p&gt;
&lt;p&gt;Your student could also read-around the whole section as well - useful info.&lt;/p&gt;
