You can either give Bayer a ring, or see if the practice has a log in for their site www.vetcentre.bayer.co.uk/ where you can see and order all of the materials you need.

Hi!

we use these already as we see quite alot of exotics.

It covers accomodation at home, cleaning regimine, diet and feeding, water provision, bathing, previous vet treatment and visits. There's a good example form in the BSAVA reptiles manual x

Bayer do a range of pre consult forms, not sure which animals they do for other than rabbits and tortoises off the top of my head.
