Do I have to formulate a written home care plan and keep just in case it needs to be verified? If so I don't know what I

RE: NPL- Home care plan for chronic patient

I just made a template and took it from there. One of mine was a vets dog with chronic liver disease and reoccurring anaemia, I talked about changes in diet, new exercise routines, medication, things to look out for, home circumstances that could affect his condition like other animals in the house with different food (the dog was a scavenger and likely to eat it as well) making sure they knew young kids in the house shouldn't feed him treats etc. Because it was a vets dog I was also able to talk it through with her as if I was discharging the patient as well. You could use something with a heart condition, kidney disease, hyperthyroid cat. Just do as many as you need to to feel comfortable you dont need to do loads.

Hope that helps
