Here is one that I have used before- not sure how up to date it is, and it had the practice details and animal details at the top. Hope it gives you a starting point anyway:

I hereby acknowledge that the discharge of the above named animal from the hospital is against the specific recommendations of the Veterinary Surgeon in charge.

It has been explained to me, and I understand, that the removal of my pet may have serious adverse effects on my pet’s health and well being, for which I accept full responsibility, both morally and financially.

I also agree and understand that this action means that the practice can accept no further responsibility for the future health and well being of my pet
