&lt;p&gt;&amp;nbsp;&amp;quot;Centres or students can apply for the replacement of damaged certificates. The request must be made in writing, accompanied by the original certificate.&lt;br /&gt;In the case of lost certificates, a request must be made using the form Application for a replacement certificate (see Appendix 4). Students and/or centres must demonstrate that they have taken all possible action to find a lost certificate before applying for a replacement.&lt;br /&gt;Replacement certificates are issued in the student‟s name at the time of original issue.&lt;br /&gt;A fee is payable for this service; see the RCVS fees list for details.&amp;quot;&lt;br /&gt;&lt;br /&gt;If you go to &lt;a href="http://awardingbody.rcvs.org.uk/document-library/operations-guide-for-centres/"&gt;http://awardingbody.rcvs.org.uk/document-library/operations-guide-for-centres/&lt;/a&gt;&amp;nbsp;and download the PDF there is appendix 4 attached that you can use to fill out and get a replacement, it is &amp;pound;40 to replace a certificate.&lt;/p&gt;
