The Handbook of veterinary nursing by Orpet and Welsh has a really good chapter on nursing care plans and includes some templates as well. We used to use a Roper, Logan and Tierney model in practice but are now using an adapted activities of living one which is much more straightforward and I feel links better to our patients. I will see if I have an example template and email it to you. Most importantly don&amp;#39;t panic they seem daunting but they are not that bad most of the time it&amp;#39;s just expanding on what you already do in your hospitalisation notes :).
