We have found the ability model care plans easier to use rather than other templates (Orem etc)

my college are very happy for us to use the ability model for all care plans . I've included the link to what I use . Does have `mug training` on but again our college is fine for us to use it!

http://www.google.co.uk/url?url=http://www.myftraining.com/Docs/Care%2520Plan%2520(blank)%2520-%2520The%2520Ability%2520Model.doc&rct=j&sa=U&ei=venlUPrZEaiP0AWC2YCwDQ&ved=0CB0QFjAA&q=ability+care+plan&usg=AFQjCNGuKw0p-n1_LjK_En6_rSSOXKjcng

Hope this helps and good luck!

However, a nursing plan itself needs to describe the needs of the patient and any special requirements and an action plan of how you will achieve these needs and requirements to reach a goal.

So the best thing to do is adapt a hospital form so that the first page, describes the animal and its problems, the needs and requirements of that animal - i.e urinating/defacation, eating/drinking, comfort, pain management etc etc (the list is quite extensive but you do it for each patient), with you explaining your action plan of how to acheive these goals. Youc an also put on their any special requirements they have before their stay with you - type of litter, biscuits vs wetfood, some dogs won't toilet on concrete, some cats only eat at night, and how you will provide this for the animal.

Hope this helps!

I am a clinical coach, and also as I did the portfolio not too long ago I can remeber what I had to do, and I have just done a few of these with my student.
