Beaten me to it l have just posted a scan of the insert from NPA and who it is from l didnt have the address. Next time l will read the post then do the scan and save me work

We have a C D Register or Poisons Register. We enter date, name, address, particulars as to license or authority of person or firm supplied to be in posession - in thise case vet practice. Amount supplied, form in which supplied e.g. injection into muscle, vein etc

It was published by the National Pharmaceutical Association, Mallinson House, 38-42 St Peter's Street, St Albans, Herts, AL1 3NP

It is quite old so I am not sure if this will be ok any use to you or not!

You can get a dd folder and buy packs for inserts that has all the info. I will try find out who makes the one l am thinking of. Will get back to you later in the week.

It should also be a bound book too. Worked in places where it is a pieces of paper in a folder, and that as far as I know is illegal, as you can "doctor" those.

not sure legally exactly what you need but as well as what you have put we put in date of purchase / batch numbers /amount remaining in the bottle etc
