&lt;p&gt;What you describe and assess, plan, implement, and evaluate (APIE) is the nursing process; care plans are designed to help give a framework to achieve this process. &lt;/p&gt;
&lt;p&gt;For example if you are using the Roper, Logan and Tierny (RLT) model there are 12 Activities of living so if we look at the activities of eating and drinking you will assess how the patient is able to carry out this activity; can they eat and drink on their own i,e, they are indepenent or are they unable i,e, dependent. We&amp;nbsp;then plan how (if neccesary) we can make sure that this activity is carried out such as tube feeding, hand feeding etc. the nursing care is implemented (and recorded on hospital records) and when appropriate evaluated. &lt;/p&gt;
&lt;p&gt;So, for example, at the end of the day has the&amp;nbsp; assisted feeding been succesful - yes the patient is now eating then we start the APIE process again, if the patient is still not eating do you carry on with the current assited feeding or are you amending anything. &lt;/p&gt;
&lt;p&gt;Evidence base is when you use theory to confirm your practice, when you evaluate&amp;nbsp;your nursing care plans, you should look at how useful they are for example you may read a chapter in a book that says the RLT model is great, provides good structure for the nursing process whereas another book or paper may say the&amp;nbsp; RLT model is based on human nursing and many of the activities do not fit within the veterinary context.&amp;nbsp;You&amp;nbsp;need&amp;nbsp;to reference your sources&amp;nbsp;and show that you have applied these (in your care plans and nursing care) to practice.&lt;/p&gt;
