I don&amp;#39;t know if this will be of any help but one of my students in college made a hosp form and she just put pain score 1-10 in a box alongside food and water etc.
Students had to fill that box in then when they were completing their HoSP forms.
The Glasgow pain scoring seems to be pretty popular but you may only need a small box cooperated in HoSP sheet if you did it this way and make sure everyone knows that 1 is lower end and 10 is higher end.
Just a suggestion 
Others I&amp;#39;ve know have used separate sheets but then they used separate sheets for fluid therapy to.
