&lt;p&gt;I&amp;#39;m my students&amp;#39; NVQ assessor but they also have monthly meetings with the head nurse and plan targets to develop their skills etc separate to their portfolio assessments, so I asked the head nurse to write a statement detailing these meetings, examples of targets planned and met, situations where the student had initiated new working practices or dealt with disagreements etc tailored to the 5.1 and 5.2 standards, and then also set some written questions for my student to complete.&lt;/p&gt;
