Me too! Mine are somewhere filed away (helpfully) I know I have done enough but it'll be finding all the dates etc and remembering it all!

Mine are in loft somewhere too! I've tried downloading a 2011 record card and can't get it to print properly, it misses the first part where you write the date. Have tried to change size etc but no joy.

Do you know if you can request a new one, I need it for PSS inspection!

Mine too, managed to sort all mine out.....sure I've done more than I have puit down biut thankfully I have well exceeded the 45 hours needed without the stuff I've forgotton

The 2011 record card is now downloadable and you can fill it in on your computer so much easier to keep track of!!

My CPD has been recalled too *sigh*.
I have the same issue as I have moved but have luckily tracked mine down for 2 of the 3 years.
You can download a form as mentioned above and maybe fill in what you can.
Has anyone else in yr practice got a record card you can look at as you may have done some of the same CPD?

i think you can download a form from there website and if you have some idea of what you've done then fill it in and send a cover letter saying your moving house and the true copies are packed some where at the mo. Has your place of work got a record of what they've sent you on?
