Theoretically a care plan is a discussion with your vet the treatment undertaken for each individual case. How you write it down is down to the practice, we have a mixture of care plan sheets which log the animals daily actions, such as eating, drinking, urination and defecation - within that box we elaborate on any vital stats. As well we have some where to write a TPR twice a day or more in some cases TPR is required more than that. We also write down anything extra that needs doing whilst the animal is under our care, such as repeat bloods, special diet etc. How you write them down or which one you pick is down to what ever works for the practice and your staff. Hi Stuart, looks like a popular document! Could I have a copy too please? It's something I'm working on with the students at my practice at the moment. We have a couple of 'in house' (Vet4Pets) ones, which are very 'factual' based, and obviously the Ability model, but it would be good to see your adaptation of the RLT one too.

Personally I think care plans are an excellent way of prompting thought and intervention in certain areas, and I see the benefit of the way the Ability model really asks, not just how the patient is doing, but what the potential problems may be, and what you can do about them. If that encourages nurses (student, newly qualified or years in the job) to actively think about these areas, and rethink nursing strategy, then it has to be a good thing, you just have to work on developing a version that works for you in your practice.. I agree that too much writing and filling in forms is not constructive for the original goal! I've suggested that one of my students writes her own, and I'll happily work with her on that - what better way to think and learn about something than to write your own version? :)
