At that  point I noticed the trainee examiner making a mark on her sheet...so would she have missed the subtle movement if I hadn&amp;#39;t said? Unless they have changed there is no need to do them in silent right? I just spoke my way through what I was doing, each step and explaining why I was doing it. This way if I was able to rationalise the way I was doing something and it was rational but not necessarily the &amp;#39;exact precise way&amp;#39; they wanted it done, I was making it clear that I was making informed decisions, that are much more practical and much more realistic, as you would do in practice on a daily basis.&lt;/p&gt;

&lt;p&gt;There is no grid involved in our set up, and we have to place grid between animal and table if we want to use it. We have 2 grids - fitting both little and large plates.&lt;/p&gt;
&lt;p&gt;Yes I know wedges are cheap , but try telling my LA bosses that!!!!&lt;/p&gt;
&lt;p&gt;Actually I&amp;#39;ve never approached them about that! &lt;p&gt;2) the wedge helps give a true lateral of the tib/fib especially important in fracture repairs for measurements prior to surgery.&lt;/p&gt;

But would like to add that in my pracs I was presented with a load of 45 degree angle wedges and lots of big sandbags. (And that in my practice, despite our potter buckey, we only have about 6 wedges, most of which are a bit manky...lol!)&lt;/p&gt;

&lt;p&gt;You know you shouldn&amp;#39;t be using grid/potter bucky unless tissue thickness is 10cm+ or using KV of 70+ &amp;nbsp;so would be inappropriate to use when imaging this area of the body. If you don&amp;#39;t have correct wedges you could always improvise such as swabs/cotton wool etc....&amp;nbsp;&lt;/p&gt;

&lt;p&gt;I know we are lucky to have a Potter Buckey, but that is exactly my point - most places don&amp;#39;t!&lt;/p&gt;
&lt;p&gt;And that is why I didn&amp;#39;t put a sand bag over the distal limb,
because if there was a distal tib/fib #, to my mind it would be
contraindicated to place a sandbag, UNLESS there was a potter buckey! Evan then, I&amp;#39;m not sure I would in a sedated patient who was lay still.&lt;/p&gt;
&lt;p&gt;BUT I was taught at college that there must be a sandbag over the distal lib of the tib/fib # practical....... and just wanted to point out that although I didn&amp;#39;t follow the OSCEs to the letter on that task, I still got 100%&lt;/p&gt;
&lt;p&gt;The OSCE practicals are a bit far removed from real practice IMHO!&lt;/p&gt;

&lt;p&gt;All I did was go through them over and over again and then practise practise practise. Even when I thought, right I know what to do now, I did them again and again. To begin with I was making lots of mistakes but when they are pointed out to you it makes it easier to remember. We had a few laughs about it as I could not remember the word umbilicus and said belly button, haha. But I didn&amp;#39;t forget it again!&lt;/p&gt;
&lt;p&gt;Just try your best to do things that help you relax. I would revise and then go home and blast my music as this let out all the pressure I was feeling. If having a bath helps you then have lots until your exams! &lt;/p&gt;

&lt;p&gt;Personally a small wedge (15 degree max) &amp;nbsp;should be place on the underside of the tarsal joint and a sandbag placed over. &lt;/p&gt;
&lt;p&gt;1) the patient was only sedate so may move during exposure, very rare but have seen it under poor sedation.&lt;/p&gt;
