we nurses all grouped together at our place decided what we wanted and made it on word.

we have a g/a form with one side for the chart, drugs, consumables pre op check list with comments boxes etc, the otherside has a tick sheet for pre med, catheters in/out walked/fed contact details etc

we have a tpr form for the kennel nurse to continue with

hospital forms with meds and comments boxes

lots of forms but comes in handy :)

Thanks for the suggestion We use the gold standard GA charts which do have a bit of space on for post op. I've made a table which i've photocopied onto the back of the GA charts with pre op HR, RR and temp at the top then post op HR, RR, temp and comments boxes. Nothing flash but they're doing the job ok!

I think it's Vetoquinol do nice Alfaxan branded GA monitoring charts that include spaces for post op meds etc & the actual chart is quite big so should give enough space to continue monitoring afterwards...?

Just noticed the date on your last post about this, so guess you might've sorted it by now!

Yeah i looked on that thread but there aren't any charts I want to have some sort of chart for the back of our GA charts, so that more can be written throughout the afternoon if needed.
