We just use a lined book, which we title at the top of the columns. A book lasts us about 3 years i think with the number of xrays we do!

Sorry maybe I didnt explain myself!. I need a copy of a chart to put all the xrays taken onto? ie to record each xray!

It would depend upon your machine/FFD etc really. Have you got one already? Easiest thing (though time consuming) is to create your own during a series of different views and exposures and make a note of the good results. You could also see if the manufacturer has anything they can send you - when we got our new machine we got a sheet from the company and then just added to it.
