I love the alfaxan ones that Vetoquinol will send you for free

They're nothing to do with using Alfaxan, just decorated advertising it really.

As you requested... it has it's own 'folder' in Anesthesia & Pain, etc.

There are two different forms. The first 2 docs are really 1 form, front and back.

The third form is from Abbott and is free source to use.

If anyone is interested, please email me directly as I have a two sided anesthesia monitoring form, similar to university style with the grid and appropriate spaces for patient monitoring and accoutrements. It is also in MS Word format and Excel so you can adjust it for your personal use, if helpful.
