It sounds like a complicated case and would not have been an easy fix and poor prognosis anyway, particularly if it was unstable before anaesthesia.&lt;/p&gt;
&lt;p&gt;I would recommend doing a Morbidity &amp;amp; Mortality meeting (if you don&amp;#39;t already do these) and you can talk through cases like this with your colleagues, particularly staff involved with the case. They are &amp;quot;no blame&amp;quot; meetings and simply there to discuss cases of near misses or deaths, and see if there could be any changes implemented in practice. Always good to ask questions and reflect on these situations. For example; Could the drain have been placed under local rather than unstable dog under GA? Was free fluid in abdomen putting pressure on the chest? Was his chest raised? Is there a ventilator available, could this be considered in future? Was he on capnography? What drugs was he given - would they have affected respiration?&lt;/p&gt;
