Would anyone be happy to share a copy of their cat scratch form that they give to clients in the event of a cat induced injury? I would be super grateful if anyone could share!
Many Thanks,

CLIENT INFORMATION
URGENT ACTION REQUIRED
Cat Bites & Scratches, Medical Implications
Client Name:
Wound inflicted at on:

We strongly advise you seek medical attention as soon as possible and present this
note to your GP or A&E department.
Cat bites and deep scratches are potentially very dangerous. Most normal cats carry a
variety of aerobic and anaerobic pathogenic bacteria in their oral cavities, including
Pasteurella multocida, Streptococcal species and Fusobacterium organisms. Pathogenic
bacteria can also be harboured in the nails and introduced through cat scratches.
Bartonella species, now considered to be the cause of Cat Scratch Disease, can be found
in a significant percentage of cats.
A bite or severe scratch can result in deep implantation of infection. Particularly when a
joint is involved, inadequate treatment can result in permanent disability. Cat Scratch
Disease can affect a variety of organ systems with potentially very serious
consequences, especially in immuno-compromised individuals. Those with prostheses or
other implants are at particular risk of serious consequences from infections arising from
animal bites and scratches.
Early antibiotic therapy is usually required to prevent the debilitating effects of such
injuries.
