Have the company name and the title of the SOP at the top

If you can reference any part of the SOP to some evidence, or off the back of a clinical audit that's always a good thing - or at least provide a rationale for what you are asking people to do

Make sure to include the date that the SOP was written, and a date for review (usually a year)
