&lt;p&gt;&lt;br /&gt;The aim of the game is to not disturb from their usual routine as much as possible. Where I work they will often get their meal and insulin in the morning (Os usually do it 6/7am) and then anaesthesia about 4 hours from there. If you use shorter acting drugs and doses to effect (as well as local blocks! See &lt;a href="https://docs.wixstatic.com/ugd/a11d90_05a95cd13e324bc4bd9079cb465d1045.pdf" target="_blank"&gt;here&lt;/a&gt;&amp;nbsp;for some) then they will be awake in time for dinner!&lt;br /&gt;&lt;br /&gt;But if they are an early morning procedure, we don&amp;#39;t ask that they&amp;#39;re fasted and then given a &amp;#39;half dose&amp;#39;. They are fasted, BG checked, and THEN insulin dose is provided IF NEEDED.
