I like the High ASA form - would probably use it for all anaesthetics despite whether the patient had a low ASA score or a high one. It's very similar to the one used in my practice.

Though, just because I'm picky, I would prefer all the o2 levels/agent levels/SPo2/ETCo2/temperature to be at the top of the chart. Also maybe squeeze in a crash drugs chart?

You can just write the volume of the cuff in the box? It's quite large.

You shouldn't be beholden to a form. I've never met a GA form I didn't customise in some way and the AVA one I have has pretty much everything you've asked for? a best to have a template you can make your own and keep thinking than become stuck because "their isn't a box"?

Yes but I think volume in the cuff is important, not just cuffed and uncuffed. Also I will lubricate multiple times throughout an anaesthesia. I guess I could just write it as an 'event'.

On the copies I have they aren't specified high or low and have boxes for all that info?

And boxes for cuffed/uncuffed, warming info, eyes lubricated etc?

In the low ASA UK form, I think it would be good to have a temperature monitoring section and an ETCO2 box to chart too. I see this is on the high ASA level, but I think good for the low too.

I usually pencil in my charts a RL LL S or D for positioning, if the get moved about through the procedure.

I also write ET tube size, in/out time, cuff volume inserted and depth inserted. Am I too OCD?
