when we've had sheets sent over with patients from other practices the things we tend not to be able to gather is, iv catheter care, when was it placed, has it been properly maintained etc?, when was it last fed? what was it fed? routes of drug administration? is there a procedures slot? for patients with urinary catheters then was the collection system last emptied? what's it's current urine output? similar story for patients with abdominal drains

it really depends on what you need from the sheet for your practice, we currently have hourly sheets as standard and icu sheets with 15 minute obs slots which are very similar to the treatment books you see in human icu, we usually only use them for very intense patients.
