Im wondering if anyone could help me with a situation I had on Tuesday. We had a 40kg labradoodle in for lump removal. After induction she was hooked up to our circle system in prep. Everything was fine, breathing normally etc. After clipping

RE: Humphreys ade system

Thanks for all the responses. So I think it was a combination of quite a few factors on this particular day.

I have since discovered that I definitely need to pre-fill the circuit before connecting the animal. My rebreathing back was too big (I had a size 3 bag attached- dog was 49 kg) and I actually only needed a size 1 or 2. My soda lime was almost ready to be changed over and I think her flow rate was a teeny bit low to start with, therefore increasing the co2 and causing the panting. coupled with not flushing the circuit through and her not getting enough iso in the beginning this all made for quite a stressful anaesthetic.

The circle had just been used before by another patient so was therefore already flushed, hence he settling on the circle. I have since been very regimented with using the ADE and it had been great. But you really do have to follow the rules!

RE: Humphreys ade system

When using low flow rates you need to be careful that your vaporiser is calibrated to be accurate at low flows. Tec 3 vaporisers which are probably the most commonly used in veterinary practice at not designed for low flows and their output may be inaccurate below 750ml.

Hope that is helpful

Also, the valve must be open at all times because that can sometimes really effect breathing. When you first moved the induction flow rate should be 1.5l/min but once settled you can reduce to 500ml unless using a capnograph (in which case 700). Once you get used to the ADE you'll never look back!!

RE: Humphreys ade system

Just a thought... what anaesthetic Vapour are you using. As I loved Sevo over iso, As you can prejudge certain pain responses to stimuli. such as the cutting of the cervix etc. Which you then increase the percentage of sevo for a few seconds prior to this. You will also notice your anaesthetic chart recordings are much more level.

RE: Humphreys ade system

I agree with the above post - it takes ages for them to fill up (although strange how she settled on the circle so quickly) - was anything painful happening at the time?

Try getting someone in to prime the ADE for a good few minutes before you connect the patient wherever possible :)

RE: Humphreys ade system

Everyone will have had a simlied situation when changing over machines that use the circile..

I am its to do with changing from a circuit that has a build up of anaesthetic gas within (achieved during the first stage of induction and higher flow rate) then moving to an chamber that has not been used,,
