Hi Gemma,

The Association of Veterinary Anaesthetists has two great ones. One for routine cases and one for more complex/higher ASA case. They have been designed by anaesthetists and nurses from both specialist and general practice.

It it a free resource that can be downloaded from the website and the template allows you to put your practice logo into it.

There are also safety checklists and a booklet about how to implement it.

Hi Rachael, take a look at the one I designed. Used in a couple of practices now. You can download it from the LINK.
