I've been charged with designing new anaesthetic monitoring charts for my practice.
The Association of Veterinary Anaesthetists has two great ones. One for routine cases and one for more complex/higher ASA case. They have been designed by anaesthetists and nurses from both specialist and general practice.

It it a free resource that can be downloaded from the website and the template allows you to put your practice logo into it.

https://ava.eu.com/wp-content/uploads/2015/11/AVA-ASA-Low-Electronic-UK.pdf

There are also safety checklists and a booklet about how to implement it.

Hope that's helpful
