&lt;p&gt;On the face of it, a relatively simplistic document, but in my mind the culture of documentation should ideally be that in a years time people would likely check the computer record and not the paper one; therefore lengthy observations and notes are better on the computer record and shorthand on the sheet.&lt;/p&gt;
&lt;p&gt;In the AM/PM column you would write the time of your obs and then go across from there. Daily requirements is where you listed medication,dose, how frequently and route, along with any care notes and feeding instructions. In the medications column you would write the medications in chronological order from the top down. It is all designed by my own hand and you are free to use and abuse as you see fit :D&lt;/p&gt;
