I dont have a copy of my clinics discharge notes but the layout is if the animal has had a sedation or GA we explain about the animal bring sleepy when they get home, feed bland food etc normal post ga client info. We then list what meds the patient is on and what is due when and explain if its pain relief or antibiotics etc. We have a special instructions section i,e bandage care, exercise routine etc and any nites from the vet in charge of the case then when the cleints needs to be seen again and a section so we can write derails of appointment booked.
