&lt;p&gt;Thank you for replying, I really don&amp;#39;t know why I get so hung up over these care plans lol. I failed my assignment and this was because my careplans were not right so just panicking now about how to do them. I know I can do them.&lt;/p&gt;

&lt;p&gt;You need to think about &amp;#39;activity of living&amp;#39; in one part of the table. Actual problems, potential problems, nursing action, review time and any changes. this works quite well in a table. Have you looked at Orpet and Jeffery?&lt;/p&gt;

&lt;p&gt;hope you don&amp;#39;t mind me asking, I am struggling with care plans especially for my assignment. I really do not know where to start with them or how to word them. do you know where I can find a better example of a filled out nursing care plan for a surgical patient and medical nursing apart from in the nursing book (pink One). No one in my practice has ever had to do care plans so not able to help much.
