Does anyone have a good chart/calculation sheet for working out CRI dosages/rates for Methadone, Lidocaine and Ketamine that they wouldn't mind sharing?
We haven't really used this before but after doing one for the first time the other day

Re: MLK CRI calculations

Thanks guys!

Re: MLK CRI calculations

I have used both morphine and methadone in MLKs, there is less evidence about methadone, but it is used. Some people refer to it as a MeLK to show the difference.

Only downside to above recipe is that it can be used post op without overdosing fluids or under-dosing analgesia. I would recommend making a 250ml bag up to run at 2ml/kg/hr and then using a second CSL infusion to top up to surgical fluid rates during the procedure.

Doses I have used at the RVC are

meth/morp 0.1mg/kg/hr
Ketamine 0.6mg/kg/hr
Lidocaine 3mg/kg/hr

You can also leave out the lidocaine and use the same dose of MK for cats

Re: MLK CRI calculations

Have copied from the chart we use for orthopaedics:

Comfortan 0.6ml / Lidocaine 3.75ml / Ketamine 0.3ml mixed with 495mls Ringers. Infused at 10ml/kg/hr.

Equivalent to

Methadone 0.12mg/kg/hr
Lidocaine 1.5mg/kg/hr
Ketamine 0.6mg/kg/hr

The reason for this is that there is not enough published data on the cumulative effects of methadone. So although Methdone is the licensed mu agonist, morphine can be used in this combination under the cascade. We use the MLK drips in all our orthopaedic ops and find it is very effective.
