How do you go about giving this combination? I was taught this can be a safer option in older dogs and would like to try it in my practice as we use Medetomidine

Re: Ketamine and Diazepam

Thanks everyone for your help. I'm not wild about just using medetomidine either but trying to change things slowly only recently started using bup so slowly but surely things are changing. thanks again I'll research this a little more

Re: Ketamine and Diazepam

This is a tricky one... our standard protocol is acp/bup and half dose acp for over 8. Compromised animals - often cats, will have a diaz/ket mix at a dose of 0.1ml/kg given iv for chest xrays or bronchial wash etc but this is not as an induction it as an anesthetic with nothing else apart from o2 by mask. This gives you about 20 minutes of non-invasive procedure time.

Occasionally if ACP is not an option (brachycephalics / epileptics especially) then buprenorphine is given alone as the 'pre-med' and then induction with propofol - but note the un-premeded dose of propofol is higher than the pre-meded dose due to the lack of pre-med - sorry can't remember the volume as we don't do it that often

Personally I hate the idea of just medetomidine as a pre-med especially without multimodal pre emptive pain relief on board but also as that would never give you sensible pulse ox and hr values of use but then every clinic will have different protocols. Maybe consider talking to one of the anaesthetists at one of the uni/colleges about this, they are incredibly helpful and approachable and will probably provide you with a more balanced answer.

PS- ketamine stings when given IM

Re: Ketamine and Diazepam

We use it as a co-induction method, its definately good to consider other ways of induction as long as your reasons are justified and backed up by up to date evidence. Good that you are changing protocols according to the induvidual animal.

As far as I am aware you would give diazepam first then ketamine to effect but PLEASE check that first as I may not be right!

Re: Ketamine and Diazepam

Same here, never seen propofol just on it's own.

One of our vets gives midazolam as a premed in oldies but only if nothing painful going on for GA - say abdo xray or ultrasound.

Have seem 'homeopathic' doses of ACP given too.
