I arrived on sat morning to a blocked cat which turns out he admitted at 8pm on Fri night and left overnight despite being blocked for 24hrs

Re: Urethral blockage - urgent help

What a nightmare!

I know you may feel awkward but if u r registered vn then it is your responsibility to provide the best care for your patient. I would definately call the o.o.h vet to unblock. I can not imagine how uncomfortable it must be if you can't physically urinate.

Hope the cat is doing better today.

Re: Urethral blockage - urgent help

How is the cat today?

Re: Urethral blockage - urgent help

[quote user="Gillian Mostyn"]

Yes - which is why I took my end comments to a private message.
Yes and this is a colleague of ours who is alone and desperate for help, in an impossible situation, faced with a poor cat who is most likely feeling very uncomfortable and is restricted by the fact that we are "just" nurses and can't be the ones to direct a course of treatment. I have been in a very similar situation, it is incredibly upsetting and frustrating and I know you won't get a wink of sleep tonight you poor thing. Call the locum. He may well get p***ed off, but I would calmly and politely explain that you are sure that the protocol in THIS practice would be to at least ensure the bladder is decompressed, and that your BOSS would probably take that course of action. He may take the hint.

Its all very well to suggest that this colleague addresses the problem when her boss returns, but it doesn't sort the extremely pressing problem of a cat who is in all likelihood very uncomfy right now. At the end of the day, if the bladder is enormous it needs to be emptied.

Best of luck, be brave xxx

Perhaps write your concerns down so you can address them fully. Maybe you should also discuss the case with the vet on call to voice your concerns regarding the treatment provided before you address it further. Certainly contact him if you feel the cat needs further attention.

Re: Urethral blockage - urgent help

have sent you a PM

Re: Urethral blockage - urgent help

do you have no contact available with your regular vet at all? not a good situation at all. But you still do have to work with this vet so can you perhaps explain to him your concerns in a non confrontational way. Its no good getting annoyed with him as it wont help the cat at all or if you have had any similar cases in you can think of can you perhaps direct him towards their case histories and explain that is your practice's protocol?

if it was me I would leave it to the vets between them to decide what they are going to do. If you are conecerned about the cat then yes relay that concern to your boss but be prepared to take any flack for doing so and then leave it at that

It is irrelevant that the locum vet is getting paid 3x as much as you - wage doesnt come into this, he is a vet and you are a nurse, leave it to the vets to decide the course of action between them it is neither your job or your place to be directing treatment.

Re: Urethral blockage - urgent help

I would have a chat with your boss if I were you so that they are aware of the situation.

Re: Urethral blockage - urgent help

I'd trust my instinct....sounds like the ooh vet should be getting a call. 24 hours is a long time without peeing, esp if he's been on IVFT x
