Merial supply them, they are sponsored by Previcox and they have the monitor chart for you to plot the readings, space for pre-meds etc and they is a section for pain scoring the animal and also a section for post op monitoring. They are made up ready for dogs but we use them for cats aswell.

We use the gold standard ones, I really like them. You can get a pad of them...can't remember which company they are from, anyone else remember? Phizer maybe?
