&lt;p&gt;The consent forms used at my current practice really need updating, but since I am the only nurse at the practice, there&amp;#39;s no-one to bang heads with and I&amp;#39;m a bit lost as to what the gold standard is.&lt;/p&gt;
&lt;p&gt;&amp;nbsp;&lt;/p&gt;
&lt;p&gt;Currently our consent forms ask for/contain:&lt;/p&gt;
&lt;ul&gt;
&lt;li&gt;owner name/add/phone no. (3 slots for contact no&amp;#39;s)&lt;/li&gt;
&lt;li&gt;patient signalment (with a space to write down current weight once admitted)&lt;/li&gt;
&lt;li&gt;confirmation that the person admitting is the owner or authorised agent for the animal&lt;/li&gt;
&lt;li&gt;authorisation for the use of drugs&lt;/li&gt;
&lt;li&gt;understanding that anaesthetic techniques/surgery involves some risk&lt;/li&gt;
&lt;li&gt;understanding that positive results cannot be guaranteed&lt;/li&gt;
&lt;li&gt;understanding that we may need to extend/alter the agreed-to procedure due to unforseen circumstances but that every effort will be made to contact the owner prior to providing unanticipated treatment/care&lt;/li&gt;
&lt;li&gt;authorisation for the vet to use their professional judgement regarding the above point&lt;/li&gt;
&lt;li&gt;consent/authorisation for the procedure (where there is a blank space to give details on the procedure - there is no continuity as to how these spaces are filled by different staff)&lt;/li&gt;
&lt;li&gt;query as to whether a pre-op blood sample is to be taken/tested&lt;/li&gt;
&lt;li&gt;understanding that fees are due at time of collection (I add a rough estimate in here)&lt;/li&gt;
&lt;li&gt;&amp;quot;I have read and understood this form and hereby voluntarily give my consent&amp;quot;&lt;/li&gt;
&lt;li&gt;Then a space for the owner/agent to sign and date&lt;/li&gt;
&lt;/ul&gt;
&lt;p&gt;Any thing else to be added?&lt;/p&gt;
&lt;p&gt;Maybe asking to add their pet insurance company?&lt;/p&gt;
&lt;p&gt;What about a place to record the pet&amp;#39;s items brought to the surgery?&lt;/p&gt;

&lt;p&gt;Thanks for that. For the consent form im just wondering what&amp;nbsp; people are using generally in practice. We have a page that outlines procedure,&amp;nbsp;risks etc but Im also wondering if most practices&amp;nbsp;highlight areas to be clipped, discussion on that specific procedure etc.&lt;/p&gt;
&lt;p&gt;We are looking to add more detail to our one so Im just looking for more ideas etc.&lt;/p&gt;
