Hi there,

we routinely pre-med with acp/vetergesic and metacam ( if appropriate) and couldnt recommend enough Alfaxan instead of Propofol.

Not sure what to say about the more fractious kitties but we don't really use ketamine a whole lot so I'm sure you should be okay without it.

We've had 7 bottles of ketamine delivered today from Dunlops.....our back order list! Just asI wasgoing to sort out an SIC!!!

I ordered Ketamine 1000 (Virbac I think) with a Special Import Certificate from France from the VMD a couple of weeks ago through Centaur - came the next day....

Get a SIC off the VMD website and then phone Virbac who will supply Ketamine 1000 from france to you through your wholesaler. It came within a few days

unfortunately Narketan is also on back order as is ketaset and vetalar - but good idea tho its the same as happened with lepto vaccines - one vaccine goes off the market for a time and other brands also go off one by one because nobody can keep up with the demand

in practice we use dom/torb then isoflo, we dont place i/v catheters in

I'm not sure if it will also be affected but we use a product called Narketan which is ketamine but a different brand - might be worth seeing if this can be used as an alternative?

Everything gets acp/bup and propclear, if they are fractious (or cat castrate), they get sed/bup (or if this is noted post pre-med, they get just seditor), gas then intubate ASAP. Often can be maintained with a sniff of gas (as they remain sedated, then reversed and woken up at end of procedure) Don't use Ketamin anymore, 28d rule means on the odd rabbit case we'd see we'd end up throwing it away 28days later! Not cost effective!
Throwing stuff away makes me cry anyway.
I think intubation is just as important as placing an IV, ever tried intubating to get a patant airway after the animal has gone a non-healthy shade of purple? It's hard when your in a slight panic.

We put an I/V catheter in all but the most fractious patients, premed with vetergesic/ACP and induce with propofol. Previous posters are so right in saying an I/V catheter is less time consuming than finding a vein in a crashed patient; I think your vet is being either plain lazy or not organised enough in not placing an I/V, especially for a spay when a whole organ system is being removed.

Did speak to Virbac rep today and they are bringing out their own version. She said she'd get her product manager to ring me back when happening......

yes I think it was Dunlops who gave us the details to get a special import on this.

Me too Claire! Most of the vets I work with now, have realised it's easier to just place an IV and keep me quiet, than try to induce off the needle!

We have a different brand of ketamine at the moment (normally use Vetlar, can't remember the new one)...used it for a pair of feral cats today, and will be doing so tomorrow as well. Very useful for them!

I have a huge thing about having lines in anaesthetised patients, have seen too many routine procedures crash in my time as it's "too time consuming" to place an iv.

Just because the procedure is routine, doesn't mean the anaesthesia will be.

We generally use buprenorphine & ACP pre med, then IV catheter & propofol, ET tube etc, for all but the most fractious patients.

Very occasionally use triple - one vet uses dom/torb pre med, & just a sniff of propofol to induce a VERY sedated cat- most cat castrates don't even need tubing after that, just O2 via a mask, tho the spays need to be tubed and iso/o2

TBH there are a multitide of GA protocals, all with their benefits and drawbacks. Dom/Torb IM knocks most cats pretty flat though in my experience, enough for all but the most fractious of cats to allow IV access - in which case we would probably get the crush cage out! But I have seen that dom & torb does the job.
